General Information
Job Title: Health Navigator
Department: Research and Development
Reports to: Director of Research and Development
Contract length: 1 x Contract Position until March 31, 2027. Benefit eligible on a 6 month or longer contract
Job Summary
Based out of our drop-in centre, a Health Navigator provides support to and supervision of those visiting the centre, including connecting them to testing, supports, and resources, meal preparation and service, and relationship building. The successful candidate will have experience navigating health and housing systems for vulnerable populations, running programming or support groups, food security, and outreach.
Key Responsibilities
- Support people in the community with the uptake of Harm Reduction practices
- Connect people with traditional Indigenous medicines and ceremonies.
- Provide Harm Reduction materials.
- Provide testing and automatic referrals to treatment.
- Provide referrals to internal programs and external partner organizations.
- Provide or arrange transportation to street-involved people to attend other services or make it home safely.
- Connect with housing and income support programs.
- Refer to other harm reduction or treatment services as identified by the client.
- Provide information on HIV/HCV/STIs and Harm Reduction.
- Organize and lead education, social and emotional support groups, as well as social events designed to education, break isolation, and promote resiliency.
- Work as part of a multi-disciplinary team
Education & Training
- Post-secondary education in Human Services or a minimum of two years of relevant experience
- At least two years of experience with the specific population serviced by our agency
- Previous experience working with Indigenous peoples and community, and a good understanding of Indigenous history, cultures and knowing, being and doing.
- Must have Glass 'G" Driver’s License and clean drivers abstract
- Experience with food handling/meal preparation and Safe Food Handling certification would be an asset as we serve breakfast and lunch daily.
Experience
The Health Navigator must have a thorough understanding of the client population we serve and the goals of our programs (e.g., harm reduction, HIV, HCV, STIs and addiction). The health navigator should have experience working with diverse populations and responding to cultural and social issues while maintaining client respect and dignity.
Demonstrated competency in the following:
- Communication
- Interpersonal effectiveness
- Maintaining Confidentiality
- Data entry into the orgs EMR
- Ability to receive and send emails and texts
- The use of an insured motor vehicle for work-related purposes would be an asset
Job Types: Full-time, Fixed term contract
Contract length: 20 months
Pay: $25.62 per hour
