REPORTS TO: Ubuntu Manager

HOURS: 37.5 hours/week including weekends and evenings to meet families’ needs

This position is based out of the C5’s North East Hub – 14023 Victoria Trail.

“Despite great challenges we believe unhealthy cycles can be broken, new opportunities created and, through healing, health restored.”

The Ubuntu CSD (Collaborative Service Delivery) Program is seeking a Family Connections Facilitator; an integral role on the team responsible for working with families and youth to help them to create a safe and healthy household, to model parenting skills, develop healthy family relationships and boundaries, and to provide fully engaging family time visits. Family Connections Facilitators strive to build a family’s capacity with an emphasis on community and cultural involvement, and helping connect families and youth to long term supports in their communities.

“We stand with our clients, side by side, patiently and for as long as they want – a constant and consistent caring force”

Position Responsibilities:

  • Work in collaboration with the FSW, family members, and other service team members to develop a CSPF for the family that focuses on specific goals and reflects Ubuntu’s four pillars (children, family, community, and culture)
  • Build trusting relationships with family members.
  • Provide strength-based feedback on parenting skills.
  • Address safety concerns by providing coaching on stress and anger management.
  • Support fully engaging family time visits between parents and their children

  • Work with the service team to locate and connect family members to local community resources that will continue to work with the family following Children Services file closure.
  • Provide transportation to and from various appointments, as scheduled or as necessary.
  • Connect families to positive cultural and community activities to build their self-esteem and sense of identity as well as expand their healthy support network (attend activities with them to build relationship and confidence).
  • Advocate on behalf of families and build capacity to empower families to advocate for themselves to community agencies to help families meet their needs
  • Contribute to a positive team environment by demonstrating the values of accountability, reliability, respect, and creativity
  • BSCS is more than a workplace. It is a community where everyone helps and contributes to the whole, and there is an expectation that all employees will take part in events (during the work week and occasionally during weekends and evenings). This includes fundraising campaigns, running community meals, medicine picking etc.
  • All other duties as outlined by program manager
