Title:
Wellness Systems Navigator – Family, Child & Youth
Salary: $88,948 - $104,644 - $120,341 per year (pay range explained below)
Contract: Term for 1 year
Hours: 37.5 hours (core hours 9am-3pm Mon-Fri)
Location: Fraser Salish Region, BC, located on the ancestral territories of the Stó:lo¯ Nations – Hybrid


About the role

As Wellness Systems Navigator – Family, Child & Youth, you will advocate for and guide children and youth with complex care needs through health and social service systems. You’ll help connect them to culturally appropriate services across primary, acute, and community care, promote collaboration among service providers, facilitate workshops on substance use and mental health, and provide training and capacity enhancement to community health staff. With a strong understanding of Indigenous health approaches and protocols, you’ll provide referrals, maintain key data and reporting, and support integrated care across the region.


About you

To be successful in this role you will have/be –

Education

  • A Bachelor’s Degree in Social Work, Nursing, or a related health profession.

Experience

  • Min. 3 years of recent and relevant experience working with First Nations communities in BC, particularly with children, youth, and families, in a culturally safe and respectful manner.
  • 4-6 years of recent and relevant experience in case coordination or case management within health services, ideally involving complex care needs.
  • Familiarity with First Nations health best practices and the ability to navigate provincial or regional health systems.
  • Skilled in developing and supporting individualized health and wellness plans for children and youth with complex conditions.
  • Comfortable working in fast-paced, evolving environments with a focus on health and wellness.
  • Proficient in Microsoft Office tools (Word, Excel, PowerPoint, Project) and in mapping health or social systems.

Knowledge

  • Strong understanding of the BC health care system and the current First Nations health landscape, particularly for children and youth.
  • Knowledge of case coordination and case management practices in culturally safe contexts.
  • Familiarity with population health approaches and First Nations health and wellness needs and perspectives.
  • Understanding of trauma-informed practice and its application in community and health care settings.
  • Experience with program design, evaluation techniques, and research tools relevant to health systems.

The Pay Range is the minimum and maximum annual salary based on full time equivalent hours.
Incumbents are typically hired, transferred or promoted between the minimum and midpoint of the range based on their knowledge, skills, abilities and experience in relation to the role requirements.
The top 10% of the pay range is for the incumbents who are industry experts in the job with the combination of exceptional experience and competencies needed to perform all duties and responsibilities at a superior capability level. Internal employee compensation will be determined as per the FNHA Employee Total Compensation Policy.
