Overview
Working as a member of the inter-professional team, the Social Worker will be responsible for providing assessment, consultation, treatment and education to the patient, family members and the interprofessional team within a variety of program areas including but not limited to critical care, respirology, medicine, stroke, surgery, family birthing centre & the emergency department. The successful candidate will participate in program development and evaluation as well as actively participating in regular team meetings, liaising with community-based providers, advocating on behalf of clients and their families and participation in inter-professional patient and family conferences as needed.
The primary area of coverage for this role will be within the VENT plus integrated care team. Strong interpersonal skills and interest in community transitions/re-integration will be an asset. Previous experience within respirology and/or critical care is preferred. Previous experience providing virtual care is an asset. This role will require availability 3 days per week.
Education
- MSW degree obtained from an accredited Social Work school required MSW degree obtained from an accredited school of Social Work required.
- Member in good standing with the Ontario College of Social Workers and Social Service workers (OCSWSSW)
- Membership in the OASW preferred.
- Additional certification/training in trauma informed care preferred
Qualifications
- Knowledge of health care legislation (Health Care Consent Act, Public Hospitals Act, Personal Health Information Act), policies, programs and issues related to social welfare institutions and community services.
- Experience in dealing with ethical andlegal implications during provision of care in a hospital setting.
- Excellent crisis intervention and mediation skills.
- Exceptional interpersonal and communication skills (verbal & written), time management and organizational skills.
- Demonstrated ability to work independently and in a team environment.
- Engages in ethical conduct and strives to understand and appreciate the diversity of our patient/staff population and community
- Computer skills required; ability to manage electronic recording, workload and meet submission timelines.
- Participates in reflective practice as per College standards.
