Your Opportunity:
AMH Supported Housing is a multidisciplinary recovery-oriented team that supports individuals who are residents at contracted supported housing sites in the Edmonton area. The Social Worker II will provide case management for clients with mental health and/or addictions concerns, participate in intake activities and ensure referrals are screened and assessed from a clinical lens, and work closely with other programs and agencies to build capacity within the homes. They will work within their scope of practice to assess, coordinate, supervise, administer and evaluate the provision of mental health services and health care resources. They act as a liaison with the individual, family, natural supports, team members, contracted operators and their staff, physicians, mental health therapists, and other programs and agencies. The social worker will also provide social work support and consultation to other team members as required.

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
