This position supports clients in crisis, who have addictions, and have mental health to access supports, de-escalate, and engage in harm reduction supports, while adhering to the mission statement and policies and procedures of Homes First Society.
Job Description:
- Build working relationships with clients who have complex needs inside and outside the shelter.
- Support clients living rough (living in encampments, tents, outside) and at the shelter with complex needs to access shelter services, harm reduction supports, and treatment access.
- Support clients to develop harm reduction plans to support clients to utilize harm reduction techniques and technologies to mitigate overdoses.
- Work in conjunction with safe supply clinics, pharmacies that administer safe supply, and primary care providers.
- Work with support teams to help clients access shelters, services, resources, and housing.
- Conduct perimeter walks around shelter(s).
- As necessary accompany clients into the community to provide support and services.
- Make referrals to the addictions, harm reduction, mental health, and physical health supports and services.
- Provide harm reduction education to clients on individual and group basis.
- Ensure that client information collected is shared with co-workers on the client services team to assist in determining the clients’ needs and developing crisis plans.
- Complete and utilize assessment tools to gather information to inform the development of client support plans
- Develop, implement and monitor individual client support plans and crisis plans for clients as required
- Respond to conflicts, utilizing crisis intervention, and desecration, and prevention skills when required
- Work from a Housing First, Harm Reduction, and Trauma informed care philosophy
- Within your team, regularly visit clients and update assessments, support plans, and consents to support client harm reduction needs
- Staff will be trained to respond to overdoses using oxygen, oximeters, intermuscular naloxone, and nasal naloxone.
- Regularly complete client documentation, reports on clients, and providing data to complete reports
- Other duties as required
Qualifications:
- Minimum one year experience in related post-secondary education, student placements, or equivalent combination of education and experience
- Ability to compile information to complete reports
- Demonstrated ability to utilize case management software systems
- Highly development harm reduction skills
- Experience with conflict resolution and negotiation skills
- Knowledge of mental health and addiction issues and several years’ experience working with clients with these issues
- Experience with individuals who lack primary health care and/or struggle to maintain medication regiments
- Demonstrated experience with harm reduction approaches and knowledge of necessary information and referrals to allow individuals the option to adopt some of the approaches into their daily lives
- Expertise in navigating the criminal justice system on all levels; court appearances, probation sentences, community legal clinics, and police departments an asset
- Highly developed understanding of and ability to communication appropriate professional boundaries
- Certificate of Completion of Homelessness Learning Hub’s Harm Reduction for the Homelessness Sector considered an asset
- Knowledge of group processes/dynamics
- Demonstrated ability in Microsoft Office
- Certificates in First Aid, CPR, Crisis Prevention and Intervention an asset
- Ability to work evenings
- Knowledge of and/or experience with the homeless population, diversity issues, anti-discrimination practices and systemic reasons for homelessness
- Able to lift 20 pounds
Working in an environment where there may be smoke is required.
