Developmental Services Ontario Central West Region, administered by Sunbeam Community & Developmental Services, is the contact point for adult developmental services in Dufferin, Halton, Peel, Waterloo, and Wellington.

DSO Central West Region is presently inviting applications for:

Intake Transition Coordinator – Kitchener, Ontario

Temporary Full-Time Position (approx 17-month contact, 37.5 hours/week)

Start Date: September 2025

The Intake Transition Coordinator is the point of first contact for MCCSS adult developmental services and supports in the Central West Region with a focus on individuals 16 to 21 years. Responsible for confirmation of eligibility, referral for application package, and processing and maintaining applicant records for adults who have a developmental disability and/or dual diagnosis (mental health and developmental disability) who are seeking MCCSS funded developmental services. This position is responsible for providing resources and presentations to school boards, individuals, and families, and offering consultation to other professionals regarding transition planning and referral.

Specific job duties include, but are not limited to, the following:

  • Providing applicants, respondents and school board personnel with information regarding MCCSSservices and supports which can be applied for as well as non MCCSS funded services and supportsthat may be of interest or benefit
  • Provide clarity regarding the application process including information on how to apply for MCCSSfunded services and supports
  • Obtain and review applicants information to confirm eligibility as per MCCSS requirements
  • Processing and maintaining requested referrals through DSCIS
  • Work in collaboration with school boards to inform and direct people to available communitysupports, fee for service options, other government supports, employment etc.
  • Actively participate on internal and external committees as required

The successful applicant will have:

  • A university degree or college diploma in a related social services field (Sociology, Psychology,Autism, Developmental Services, Community Services) combined with experience serving individualsof all ages with a developmental disability in a variety of community settings and programs
  • Demonstrated understanding and respect for people of diverse backgrounds
  • Excellent verbal and written communication skills
  • Strong consulting skills
  • Proficiency with MS Office and other computer-related equipment and programs
  • Ability to work independently and as part of a multi-disciplinary team
  • Possession of a valid vehicle operator’s license and clean driving record.
  • Certification in CPI-Prepare Training would be an asset.
  • Require some flexibility including evenings and occasional weekends.
