Intake Case Manager -KFLA Youth Wellness Hub

Overview

The KFL&A Regional Intake Case Manager will be a key component of the overarching Youth Wellness Team. They will be the primary point of contact for youth entering the hub. They will provide light-touch case management support as the youth navigate the hub's referral pathways, foster connections, make referrals to appropriate services and maintain the My Wellness Passport Database. They will also be responsible for creating a welcoming space in the One Roof Drop-in (the urban component of the hub) and any rural drop-ins or pop-ups.

Duties and Responsibilities:

· Assist youth in navigating services, including conducting intakes, developing goals and making referrals to available resources in the hub and the community.

· Providing light-touch case management support to youth as they wait to be connected to services in the hub. This will include:

o Support attending appointments based on GBOs that have been developed with the youth

o Ongoing support to help youth achieve the GBOs that they have put forward

o Warm transfers to community partners and internal referrals

· Liaise with the Coordinator if additional services are required for the youth.

· Follow-up with additional referrals on an as-needed basis

· Maintain the databases (COTS and MWP) and generate reports regularly, providing training on database usage as needed. Report any data entry issues to the program's Supervisor and Coordinator.

· Support youth in booking appointments with One Roof staff and community partners, following partner intake procedures, and facilitating reminders as needed.

· Provide services at other locations within the KFLA Regional Youth Hubs network as required.

· Serve as the hub's representative in rural communities on an as-needed basis, focusing on developing a connection to services and assessing needs.

· Facilitate and work with school boards in the delivery of PreVenture programming.

· Undertake additional tasks as assigned by management.

Skills, Knowledge, and Education Required:

· Knowledge of office skills and procedures

· Excellent oral and written communication skills; interpersonal skills with ability to set and maintain boundaries

· Strong time management skills: ability to multitask

· Crisis management skills

· Exceptional confidentiality awareness skills are a requirement

· Ability to work independently and as part of an interdisciplinary team

· Bilingual in English and Advanced Level French would be an asset

· Comprehensive computer expertise in Microsoft Office and Google Drive to prepare presentations, populate spreadsheets and databases and prepare written reports

· Knowledge of Indigenous peoples, culture, and healing practices

· Knowledge of anti-oppression and inclusion, including knowledge of supporting 2SLGBTQ+ youth and creating positive spaces

· Demonstrated ability to exercise tact, diplomacy, and good judgment in responding to enquiries

· Knowledge of housing, health care, programs, and community resources in Kingston

· Experience working with individuals who use substances, have mental health issues and/or have come into contact with the criminal justice system preferred

· Knowledge of transitional-aged youth needs

· Knowledge and implementation of harm reduction strategies

· Valid Ontario Driver’s License and own means of transportation are required

Qualifications:

· University Degree or Community College Diploma in Social Services or related field

· 2-4 years of social service or mental health experience, especially with transitional-aged youth

· Administrative experience required
