About the Guelph/Wellington Community Health Van Program
The Guelph/Wellington Community Health Van team is looking for an organized, self-motivated individual for the role of Outreach Worker. This position requires the ability to work in a fast-paced environment and to adapt to ever-changing community needs.
Van Outreach Responsibilities:
- Actively engage with community members to build trust and provide support.
- Drive the Community Health Van as a part of a regular schedule.
- Build and maintain positive relationships with individuals and other service providers.
- Support system navigation, referrals, and connection to services for clients in rural Wellington County and within the City of Guelph who are experiencing homelessness, precariously housed or with complex health and social needs.
- Provide referrals related to housing, health care and other social services for clients.
- Assist clients with documentation related to their housing, social and health care needs.
- Distribute educational materials and harm reduction supplies.
- Liaising with various agencies to coordinate services and support networks.
- De-escalate brief crisis situations, making referrals when needs are more intensive.
- Ensure necessary documentation and follow up for clients of the Community Health Van program (including using Electronic Medical Records)
- Engaging with the community through various events and activities to foster relationships and enhance outreach efforts.
- Acts a peer driver when needed (provides rides to and from site locations).
- Participates in the preparation, loading and service delivery of Van
- Help maintain an organized stock of supplies, and track inventory as part of a team
- Documenting activities and providing reports, as required, adhering to deadlines
- Participates in all team functions and meetings
Qualifications
- Possess a valid G-class driver’s license, valid insurance for work purposes, and a clean driving record. Please note: In order to receive an offer of employment, the successful candidate will need to provide proof of their valid driver’s license as well as a proof of a clear driving abstract. The cost to obtain this record will be reimbursed at that time.
- Post secondary education in the social services field, or a minimum of 2 years of outreach experience with the populations that Sanguen serves (e.g those experiencing homelessness, people engaging in sex work, those who use drugs or equivalent of lived experience.
- Lived experience is considered an asset, but not mandatory.
- Have an excellent understanding of Harm Reduction and belief in this philosophy.
- Strong ability to build and maintain professional and effective relationships.
- Must have an understanding of how to work with highly marginalized individuals, and do so in a compassionate and respectful manner.
- Strong understanding of available social services and health care systems in the Guelph/Wellington region.
- De-escalation skills when it comes to conflict and crisis management.
- Demonstrated time-management, organizational, and problem-solving skills.
- Strong verbal and written communication skills and an ability to read and write legibly.
- Ability to work as part of an interdisciplinary team and autonomously.
- Knowledge or willingness to learn about Hepatitis C.
- Must be willing to continuously learn from the experts (people who use drugs) and be comfortable working with individuals who use drugs.
- Must be able to lift, lower, and carry up to 50 pounds (23kg) repeatedly.
- Data entry skills and experience with Google Apps (documents, spreadsheets), Excel, Word and e-mail.
- Ability to work some evening hours.
- Access to reliable transportation and willingness to drive the Community Health Van in the City of Guelph and the County of Wellington.
This position also requires:
- A vulnerable sector check.
- Emergency first aid (or equivalent).

Location of Work and Hours:
This is a full-time position, working 40 hours (1.0 FTE) per week. Shifts will occur throughout Guelph and the Wellington Region. These hours will be primarily during the day, with one evening required per week. Future programming may include some occasional weekend hours as service is expanded.
Compensation and Other Perks
- The compensation for this role will range from $53,040 and $58,240/year ($25.50-$28.00/hour) depending on experience and qualifications.
- 4 weeks’ vacation (prorated by FTE) which accrues biweekly.

- 15 paid sick days per calendar year.
