The Toronto Grace Health Centre (TGHC) provides specialized care and services to those individuals who require Complex Continuing Care, Post-Acute Care Rehabilitation, Palliative Care, and Transitional Care. The care and services provided at TGHC help facilitate the patient flow through the health care system and ensures recovery to those individuals who may return to their homes or to Long Term Care (LTC) facilities in their communities. TGHC operates in 2 sites – the Main site (Church & Bloor St), Harbour Light and a Specialized Care Centre (Victoria Park & McNicoll).

Position Summary:
Reporting to the Patient Care Manager of the Specialized Care Centre, the Screener welcomes all staff, patients and visitors. The screener is responsible for ensuring all screening protocols are in place upon entry/exit as per Toronto Grace Health Centre requirements. In this role, screeners will confirm staff self-screening attestation, actively conduct a health screen for outpatients and visitors.
Qulaifications:
- Grade 12 Diploma
- Customer service experience preferable
- Experience in a healthcare environment would be a definite asset.
- Ability to work independently and accurately in the presence of frequent interruptions.
- Maintain confidentiality, exercise good judgement and discretion.
- Strong work ethic, professional, diplomatic, and dependable with the demonstrated ability to adapt to changing requirements.
- Ability to workdays/evening/weekends and holidays
Main Responsibilities:
- Alert staffing team to call bells and patient requests
- Require all guests to sign in and out of the unit when visiting residents
- Assist patients & visitors with directions to destination and ensuring people are maintaining physical distancing
- Redirect and escalate any unique inquiries or requests to the Manager or administrator on call as appropriate
- Provides effective communication to clients, families and staffing team
- Assist in maintaining mask inventory at station
- Promotes the maintenance of a safe and healthy workplace
- Other duties as required
Requirements:
- Willingness to wear Personal Protective Equipment (face shield, mask, gloves, etc.) as required.
- Ability to sit, walk and stand to effectively perform essential requirements of this position.
- Ability to use e-mail for communication
