BRIEF DESCRIPTION:
Reporting to the Snaw-Naw-As Health Director, the Community Health Representative (CHR) supports individuals and families in improving their access to health services and enhancing the overall quality of care. The CHR is responsible for coordinating and delivering community-based health promotion and disease prevention programs, with a focus on achieving wellness through culturally grounded approaches.
The successful candidate will demonstrate compassion, cultural sensitivity, and respect, while maintaining strict confidentiality and exercising sound judgment and discretion when addressing sensitive health and personal issues.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
- Carry out all duties and responsibilities in accordance with Snaw-Naw-As Health Centre (SHC) policies, standards, and procedures, and as directed by the Health Director.
- Maintain strict confidentiality in all matters related to the operations, clients, and affairs of the Snaw-Naw-As Health Centre.
- Provide health care services through planned home visits, organizing clinics, and facilitating educational workshops, which may include inviting special guests with expertise on specific health topics to enhance community learning
- Assist clients by providing information about and facilitating access to First Nations Health Benefits, Plan W Benefits, and Pacific Blue Cross coverage.
- Assists the CHN in the delivery of community health services including immunization and vaccine clinics
- Does referrals to other health professionals and service providers as required
- Assists families in completing forms and ensures required documents are completed to access health services
- Acts as an advocate for families in need of health care services and encourages ownership and responsibility of the individual's health care needs
- Participates as a team member in planning community health plans and attend staff meetings
- Addresses social determinants of health including financial, housing, mental health and transportation concerns.
- Administer the First Nations Health Programs you oversee:
- Non-Insured Health Benefit (NIHB) Program
- CHR
- Diabetes
- CiCaWaTul Program and
- Other short-term funding programs as they arise.
- Collaborate with other departments, including Maternal Health, Social Development, NNADAP, Education, Child Care Centre, Child and Family Services, Elders, and others, to support and deliver community programs
- Maintains up-to-date, accurate case notes and collects records and submits program data to the Health Director
- Ensure the Snaw-Naw-As Community is informed on health issues (hold or promote attendance at health fairs, information sessions, workshops, testing, etc.)
- Performs other duties and responsibilities as assigned by the Health Director
QUALIFICATIONS:
Education, Training and Certificates
- Education and Training background in Health Field
- Grade 12 or equivalent
- CPR/First Aid Certificate
- Food Safe or willing to obtain
- ASSIST training or willing to obtain
- Must possess a clear criminal record check and vulnerable sector check
- Must have a valid BC Class 5 Driver’s License, a reliable vehicle and a clear driver’s abstract
- Up to date immunizations

- Ability to be flexible and meet changing priorities
- Completed the Indigenous and Intercultural Awareness Training
- Knowledge of First Nations Health Authority, Island Health Authority and other health care providers of the surrounding communities is beneficial
- Knowledge in NFN community and the Coast Salish culture, traditions and history is beneficial
