Lumenus Community Services is looking for a Case Facilitator who will provide case management and facilitation and broker approved SNT funds to develop an integrated service plan based on the unique goals of the client and family. The Case Facilitator will oversee the integrated support plan when children, youth and adults are receiving live-in care or are in their home with enhanced supports. All components of the integrated service plan, including financial, are working together to ensure the best interests of the client. The Case Facilitator will report to the Manager, Special Needs Team.
KEY RESPONSIBILITIES:
- Direct case management, supports accountability by facilitating service delivery that is coordinated, timely and appropriate for individual clients
- Develops integrated service plans that identify goals for service delivery
- Assesses need in establishing supported group living needs
- Assess clinical needs and works with a variety of clinicians where necessary to assist in achieving identified goals
- Provides support to families
- Assists families in identifying appropriate crisis services as needed
- Keeps informed of available community resources in relation to established goals to ensure parent/guardian can make informed decisions
- Advocates on behalf of children, youth and families in the facilitation of service provision
- Consults with community partners to review supports and services for individuals identified with a special need
- Consults with live-in treatment programs where individuals receive care
- Attends plans of care, Person Centred Plans and oversees the placement of individuals in care
- Oversees the SNT funds that are being spent in accordance with the CYFSA and SIPDDA
- Contributes to the client’s annual budget preparation/maintenance on an ongoing basis
- Complies with agency policies and procedures and quality standards
- Adheres to the organizational health and safety policies and practices along with the Ontario Occupational Health and Safety Act. This includes but is not limited to: ensuring safe workplace practices, protecting the health and safety of themselves and others and maintaining cleanliness in
the workplace

EXPERIENCE/SKILLS:
- MSW or equivalent education and experience working with Individuals with Complex Special Needs
- 5 years’ experience supporting individuals and families in social services environment preferred
- Experience in counseling, consultation and advocacy
- Experience with supported group living system issues
- A demonstrated commitment to excellence in clinical and community centered work
- Highly developed communication skills, both written and oral
- Demonstrated strong skills in time management, organization, and administrative work
- Demonstrated ability to work independently and within a team environment
- Strong assessment skills with particular emphasis on assessing risk and potential for family breakdown
- Working knowledge of the Child Welfare system and CYFSA legislation & Services and Supports to Promote the Social Inclusion of Persons with Developmental Disabilities Act and all other pertinent
legislation
- Interpersonal skills, including advocacy, consensus/team building, facilitation, negotiation and conflict resolution
- Knowledge of, and an ongoing commitment to expand knowledge of LGBTQ, racialized, and newcomer communities, as well as working with children/youth/adults with a dual diagnosis (mental health and developmental disability) and/or Neurodevelopmental Disabilities such as Autism Spectrum Disorder (ASD) and Fetal Alcohol Spectrum Disorder (FASD), and lived experience of racism, ableism, homophobia, or transphobia are advantageous
- Extensive knowledge of physical, intellectual and neurodevelopmental disabilities and their potential impact on individuals and family systems
- Conflict Resolution competencies/ experience
- Ability to understand budgeting concepts
ADDITIONAL REQUIREMENTS:
- Satisfactory clearance under the Vulnerable Sector Screening process
- Access to reliable transportation for travel required to other agency locations and external meetings (schools, hospitals, family homes, and supported group living settings)
- May work in stressful conditions, dealing with challenging situations and highly distressed clients
- Flexibility of schedule required to meet workload demands
