Primacare Living Solutions is currently looking to fill a role of a Resident Care Coordinator for our Long-Term Care Home in St. Catharine's.
In this role the incumbent will provide Social Service work support to our residents and their families.
Responsibilities:
- Providing Support and Counselling to Residents and Families
- Facilitating admissions, discharges and transfers
- Facilitating tours of the home for new residents and families
- Attending and participating in Care Conferences
- Work with all members of the interdisciplinary team
Qualifications
- Membership in the Ontario College of Social Workers and Social Service Workers
- Experience working in a similar or same role in Long Term Care
- Knowledge of applicable regulations

Pay: From $55,000.00 per year
