The Red Pheasant Health Centre is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Home and Community Csre team. The LPN will provide essential nursing care and health services to client in their homes, supporting elders and individuals with chronic or acute health conditions. Services will be delivered in a culturally safe, respectful, and client-centered manner.
Key Responsibilities:
- Provide direct nursing care to clients in their homes (wound care, medication administration, chronic disease monitoring, etc.)
- Assist with activities of daily living as required
- Collaborate with the Home Care Coordinator and interdisciplinary team to develop individualized care plans
- Monitor and report changes in clients' health status
- Complete required documentation and assessments
- Educate clients and caregivers on disease management, medication use, and health promotion
- Maintain equipment and supplies inventory for home visits
- Travel to client homes within the community
Qualifications
- Must be a Licensed Practical Nurse, in good standing with the Saskatchewan Association of Licensed Practical Nurses (SALPN).
- Current CPR/First Aid certification
- Valid Class 5 Driver's License and access to a reliable vehicle
- Experience in home care or community health is an asset
- Knowledge of Indigenous culture and traditions is preferred
- Ability to speak or understand Cree is an asset
