Edmonton Integrated Services (EIS) is currently seeking an individual to fill the position of Community Resource Facilitator (Disability Service Practitioner) in our Community Support Program.
Fast Facts:
This is a caseload management position with a focus on connecting individuals to resources in the community, advocacy, and providing hands-on support. It is a fulltime position, primarily day shifts on weekdays, but does also require a somewhat flexible schedule depending on the needs of our individuals for after-hours appointments. Travel around Edmonton and occasionally the surrounding communities may be required, with a vehicle-use budget provided. The pay for this role is between $45,000 and $50,000 based on sector-related education and experience.
Main Duties and Responsibilities:
-Developing and implement Individualized Service Plans with Individual and their support networks
-Provide effective and flexible casework to individuals accessing our services
-Liaison between other community agencies and professionals
-Maintain Individuals files in a professional manner
-Ensure Agency policies and procedures are being followed
You will bring:
-Hours/Availability: This is a Full Time position, office hours are 8:30 am-4:30 pm, however flexibility is required.
-Formal Education: Diploma/Degree in Social services, community rehabilitation, disability studies, or a related field
-Experience: A minimum of three (3) years’ experience in providing support to adults with cognitive/developmental disabilities. Experience with mental health issues is an asset.
Knowledge, Skills, Abilities:
- Ability to confidently provide frontline, face-to-face support with individuals with varying challenges (which range from multi-diagnoses to addictions and homelessness) in all age groups I would say adults as EIS does not provide supports to anyone under 18.
- Excellent verbal and written communication skills
- Intermediate computer skills
- Strong time management and organizational skills
- The ability to work independently
- Flexibility for occasionally changing hours
- Must be able to identify issues, set priorities, and implement an appropriate plan of action
- Proven ability to work collaboratively
Other Necessities:
- Must have a reliable car, with personal liability insurance, and a minimum coverage of two million ($2,000,000.00) in third party liability coverages
- Successful candidate will need to complete a Criminal Record Check with a Vulnerable Sectors Search
-A valid Class 5 driver's license issued in Canada
