Care Coordination Lead, RPN
Pay range - $22-$29
Summary: The Care Coordination Lead, RPN will lead in delivering care coordination's and outreach activities in primary care. This is a permanent full time position (4-5 days a week, including occasional weekends and evenings).
Location: Remote, with occasional in-person meetings
Responsibilities:
- Provide virtual care coordinations to patients and insurance partners
- Support care with physicians, pharmacies and allied health professionals
- Administrative responsibilities include but are not limited to: registering patients, answering phone calls, scheduling appointments, following up on consults and test results, obtaining required documentation
- Provide service presentations to clients and partners
- Initiate and maintain patient record accurately and efficiently in the EMR
- Supports management and perform tasks for service delivery
- Performs outreach calls to secure care options
- Contribute to the development of care process and algorithms
- Perform other related duties as assigned
Requirements:
- RPN registration - required
- Primary care experience - required
- Previous medical administration experience is an asset
- Excellent communication skills, ability to multi-task and willingness to learn
- Computer proficiency
- Knowledge of EMR and G-suite is an asset
- Able to work effectively in a fast-paced environment
- Strong organizational skills and attention to details
