The Transitional Housing Support Worker provides support in a residential setting for up to 25 hours per week, including some evenings and weekends, to assist men experiencing chronic illness, mental illness, and/or recovery from addictions.
Responsibilities and Duties1. Service Engagement
● Actively listen to resident concerns to foster deeper self-awareness and understanding.
● Facilitate in house resident meetings
● Provide crisis support and make referrals to appropriate services as needed.
2. Assessment
● Monitor and document incidents or changes in residents’ health, behavior, or circumstances.
● Communicate updates with staff and external agencies to ensure comprehensive service delivery.
● Evaluate social and environmental factors affecting residents’ progress.
● Take necessary steps to maintain the safety of residents, the property, and the community.
3. Service Delivery
● Education: Assist residents in developing and managing daily living skills, provide medication support and health teaching, and encourage accountability and autonomy.
● Case Management: Facilitate personal goal setting and recovery planning, track changes in residents’ needs or behavior, and ensure a safe and supportive living environment.
● Outreach: Share information about Ryandale as needed, coordinate with other organizations to enhance resident support, and identify service gaps.
4. Organizational Capacity
● Participate in tasks assigned to further the mission and goals of Ryandale Transitional Homes.
5. Organizational Responsibility
● Engage in consultation and debriefing with house managers, support workers and administrative staff.
Ensure timely and accurate documentation of events and resident interactions

Qualifications
- Proven experience as a Transitional House Support Worker or in a similar role
- Strong supervisory skills and experience managing teams
- Excellent communication and interpersonal abilities
- Knowledge of support services and community resources
- Ability to handle crisis situations with composure
- Understanding of mental health and addiction issues
- Certification in First Aid and CPR is preferred

Pay: $22.00-$24.00 per hour
