Job Posting: System Navigator – Full Time Permanent

Organization: Lakehead Nurse Practitioner-Led Clinic (LNPLC) Location: Thunder Bay, Ontario Salary: $60,000.00 - $75,000.00 annually -based on experience and qualifications. We offer extended health care benefits, HOOPP pension (optional), 4 weeks paid vacation in the first year as well as learning and developmental opportunities.

Hours of work: Monday to Friday 8:30 am – 4:30 pm, availability for flexible hours

Position Summary:

Our Clinic is seeking a dedicated and compassionate System Navigator to support individuals with their primary care needs, this position involves case management, support, and guidance, helping patients navigate their primary care experience. The System Navigator will be a trusted point of contact, offering personalized guidance, education, and improved access to health care services and other resources to provide essential system navigation and support services to LNPLC rostered patients in Thunder Bay.

Key Responsibilities:

•Represent LNPLC with professionalism, aligning with the organization's mission and values.

•Employ a person-centred approach in all interactions.

•Coordinate care and connect patients with health care providers and community resources.

•Complete and file program application forms for funding, medical, service applications, etc.

•Assess patient needs and match them with appropriate health care services.

•Develop and implement individualized care plans.

•Facilitate continuity of care, including referrals, follow-up calls, along with providing primary care provider with updates on a continual basis in a timely and consistent manner.

•Encourage and support self-management of chronic conditions.

•Build and maintain relationships with community partners.

•Participate in program research, data collection, and evaluation.

•Ensure compliance with legislation, standards of practice, and confidentiality policies.

Education, Qualifications, and Skills:

•Diploma or degree in a health care field.

•Regulated health professional.

•Minimum 3 to 5 years of experience in community health.

•Experience with social determinants of health.

•Awareness of and commitment to community health.

•Ability to work with diverse communities with respect and cultural understanding.

•Experience in building and maintaining relationships with community partners.

•Excellent interpersonal, communication.

•Strong time management and prioritization skills.

•Proficiency in computer and word processing skills.

•Ability to work independently and as part of a multidisciplinary team.

•Current BCLS.

Additional Job Requirements:

•Satisfactory criminal records check, including vulnerable sector screening.

•Valid Class “G” Driver’s License and access to a personal vehicle.

•Up to date on vaccinations (including MMR and Covid) – must provide immunizations & Covid vaccination record
