Cota is an accredited not-for-profit organization that has been providing community-based support to people living with mental health and cognitive challenges for over 50 years. At present, our wide array of services delivers much needed support to over 4,000 people/year who live with mental health challenges, psychogeriatric mental health conditions, acquired brain injuries, developmental disabilities, dual diagnoses and/or the experience of homelessness. These services include, but are not limited to, case management, supportive housing and adult day program services. Role Overview

The Supervisor, Groups and Supportive Housing (the Supervisor) role is a leadership position within assigned supportive housing sites and is the lead for Cota’s Facilitated Groups. The Supervisor reports to Director, Toronto West Services. While the Manager, Community Health Services (the Manager) maintains overall responsibility for ensuring the Supportive Housing program achieves its mandate, service targets and intended outcomes, the Manager relies on the support of the Supervisor to do so. Furthermore, the Supervisor will be responsible for leading and supporting a team of facilitators to deliver structured and drop-in peer support groups, while coordinating community of practice (CoP) meetings.

As is expected of all leadership personnel at Cota, the Supervisor is to work within a continuous quality improvement framework, which reflects Cota's vision and mission of services, while promoting and maintaining positive relations with clients, Cota staff, partner agencies, funders and other stakeholders.

About the Team

Cota Facilitated Group sessions offer supportive and creative group environments for Cota service users to engage in meaningful activities, explore strengths, and develop new and diverse skills.
The purpose of the facilitated groups offered by Cota is to:
  • Enhance skill development and provide valuable information on a range of topics
  • Expand participant knowledge and strengths to better support their recovery journey
  • Empower participants and encourage social interactions/connections
  • Designed to act as a stepping-stone to personal recovery and community inclusion
Groups include:
  • Anger Management
  • Depression
  • Anxiety
  • Self-Esteem
  • WRAP (there should be at least one facilitator who has been certified to facilitate the group)

Primary Responsibilities:

The Supervisor’s primary duties may involve, but not necessarily be limited to:

Operational Support:

  • Working collaboratively with the Manager, the Supervisor contributes to the development of the program’s operational protocols and procedures (e.g. intake procedures, move-in schedules, unit safety checks, site visits, medication management protocols, client support, crisis management, staff supervision as required, etc.) and may assist the Manager in orientating bargaining unit staff to these protocols and procedures
  • The Supervisor is expected to assume overall operational oversight of the program during their assigned hours/shifts, when the Manager is not present
  • Assumes responsibility for ensuring that any emergency/crisis situations that may arise during their assigned hours/shifts are effectively addressed, while making every effort to safeguard the safety of tenants and staff in the process
  • Coordinates the submission of requisitions and/or work orders for program supplies and as needed, repairs to program equipment/facilities
  • Participates, coordinates and leads team meetings and/or task forces as required
  • Participates and assists in investigations relating to incidents, occurrences, deficiencies noted in quality assurance reports, and/or compliance breaches/violations
  • Assists the Manager, as required, with daily shift planning meetings, and team meetings and sessionals

People Management:

In the managers’ absence they would assume:

    • Primary responsibility for the day-to-day performance management of all bargaining unit staff working on the shifts corresponding to the Supervisor's assigned hours/shifts, and
    • primary responsibility for leading the annual performance appraisal process for the part-time and casual bargaining unit staff who work on shifts corresponding to the Supervisor's assigned hours/shifts and may also lead the annual performance appraisal process of other bargaining unit staff, as may be requested by the Manager.
  • Ensure all staff are trained in standard administrative practices, program protocols, safety procedures and applicable policies during orientation and ongoing.
  • Ensure all staff are booked and complete mandatory training and have their certifications up to date including QAM annual reviews.

In collaboration with the Manager:

    • Directs, supervises, supports, mentors and coaches the work of bargaining unit staff, while adhering to terms of the Collective Agreement in doing so
    • Organizes and coordinates staff schedule(s) including support with payroll administration and attendance management as required
    • Adheres to other terms and conditions of the Collective Agreement

Clinical Support:

In collaboration with, or as may be directed by the Manager, the Supervisor may be responsible for providing clinical support in areas such as, but not necessarily limited to:

  • Supporting staff to organize and conduct case conferences and coordinated care plans for specific tenants
  • Ensuring staff complete applicable clinical assessments, safety plans and service plans with/for tenants
  • Ensuring each staff member’s required client electronic health record entries and required data entries are accurate and entered in a timely manner
  • Ensuring documentation of client contacts/interactions are in the appropriate client electronic chart is completed by staff in accordance with Cota's documentation guidelines and applicable policies
  • Reviewing intakes for eligibility
  • Reviewing discharged client documentation prior to forwarding to Health Records
  • Supporting staff in their ability to plan, coordinate, deliver psychoeducational and/or social group activities (including addictions/harm reduction), on-site programming and community development
  • Ensuring that the program offers opportunities for family engagement in tenant goal planning processes, if the tenant wishes and consents
  • Monthly Tenant Meetings
  • Supporting Cota partnerships (i.e. Parkdale CHC)

Group Facilitation:

  • Lead and support a team of 11 facilitators (case managers and peer support workers) to deliver structured and drop-in peer support groups
  • Chair community of practice (CoP) meetings every 3 weeks and record and distribute meeting minutes with CoP and upload to teams
  • Assist with problem solving when issues arise in group facilitation
  • Confirm group session dates with the team
  • Ensure at least one WRAP-certified facilitator is present for WRAP groups
  • Update CoP chart to reflect current groups schedules
  • Advertise groups internally and externally; revise promotional material as needed
  • Liaise with communications to inform them of new or updated group sessions for inclusion in the organizational calendar
  • Collaborate with other programs and external partners to increase enrolment and development of new groups

Other Responsibilities:

  • Observe Cota rules and regulations, policies, procedures, practices and current legislation, in particular, the Ontario Human Rights Code
  • Comply with all Health and Safety policies and procedures to ensure every reasonable precaution is taken for the protection of workers and service users, and that health and safety concerns or incidents are responded to promptly
  • Works cooperatively, respectfully and constructively with service users, Team members, management, other service providers, and external stakeholders.
  • Ensure that the confidentiality of service users, employees and Cota information is always maintained.
  • Provides strong leadership as part of Cota’s Clinical Leadership Team (CLT), participates in Cota’s Leadership Forum and ad hoc committees as required
  • Undertakes other related and/or delegated duties, as assigned

Qualifications:

Education and Experience

  • Degree/Diploma from a recognized University or College preferably in a related discipline OR, a minimum of 3 years of relevant education and experience
  • Experience working in a unionized environment
  • Experience working in healthcare, not for profit and/or social services organizations
  • Experience working and supporting Peer Support Workers
  • Proven experience as a Group Facilitator, communities of practice, or similar role
  • Experience in planning and implementing group activities
  • Regulated Health professional designation (Nurse, OT, SW, Addiction specialist, Behaviour Therapist) would be considered an asset
  • Knowledge and experience in Adult Education principles are an asset

Knowledge & Skills:

  • Sound knowledge of clinical issues relating to the target population being served within the program
  • Knowledge of Recovery Oriented and client and family-centered care, harm reduction and anti-racism anti oppression practices
  • Knowledge of Electronic Health Record management, OCAN and OPOC reporting
  • Knowledge and experience supporting people with substance use challenges from a recovery and harm reduction approach
  • Knowledge of group facilitation techniques and methods
  • Strong leadership, management and supervision skills
  • Excellent interpersonal, communication, and conflict resolution skills
  • Demonstrated understanding, sensitivity to, and awareness of, issues relating to diversity and cultural competency
  • Ability to train, lead, motivate, coach, and develop individuals and teams.
  • Calm, able to work under pressure and thrive in a dynamic clinical environment
  • Effective organizational and time management skills
  • Competence in computer operating skills including Electronic Health Records
