Position Description

UNIT PROFILE
The Substance Use Service (SUS) works under the umbrella of the Concurrent Disorders Capacity Building Team and in strong partnership with the RAAM Clinic and the Concurrent Disorders Outpatient Team.
The Substance Use Service team (SUS) is a multidisciplinary team made up of physicians, nurses, a mental health worker and peer support providers (individuals with lived experience). The team supports individuals presenting to the emergency department and those later being admitted to hospital who use substances. The team also provides follow up care for individuals who have discharged hospital and works alongside individuals to connect them to outpatient care; hospital and/or community-based health care services. The team operates from a harm reduction lens that allows patients the space and autonomy to make their own choices regarding both their substance use and their health choices. Services include assessment, consultation, shared care and capacity building across St. Joseph’s Healthcare Hamilton and community partners.

POSITION SUMMARY
The role of the Peer Support Provider is to provide formalized peer support for individuals accessing hospital-based care who use substances. In this role the Peer Support Provider will see patients in both inpatient and outpatient settings focusing on harm reduction and increasing access to health care services. As part of this role, you will be working both in the hospital and community, which will include seeing patients in shelters, housing, encampments, and community serving agencies

QUALIFICATIONS
  • Minimum 1-year related experience (volunteer or employed) providing peer support in a harm reduction, health care, treatment, or community setting.
  • Formal training in Psychosocial Rehabilitation (PSR) an asset. However, will provide training.
  • Personal lived experience of Substance use.
  • Demonstrated knowledge and ability to apply the core values and principles of peer support practice including but not limited to hope and recovery, self-determination, empathetic and mutual relationships, dignity, respect, social inclusion, health and wellness and personal growth.
  • Demonstrated ability to role-model strong self-care and self-management strategies in recognition of the need for health, personal-growth, and resiliency in peer support work.
  • Familiarity and understanding of substance use, harm reduction (values and principles) as well as community resources for people who use substances.
  • ASIST or similar Suicide Intervention Training (preferred, however will be provided)
  • Experience/training in group facilitation (preferred)
  • Excellent assessment, crisis intervention, problem solving and organizational skills
  • Excellent interpersonal, communication and organizational skills and demonstrated ability to function effectively in a team
  • Demonstrated ability to be flexible in work assignments
  • Demonstrated commitment to personal professional development
  • Ability to work effectively under pressure
  • Good attendance and work record are essential
