Position Description
UNIT PROFILE
The Substance Use Service (SUS) works under the umbrella of the Concurrent Disorders Capacity Building Team and in strong partnership with the RAAM Clinic and the Concurrent Disorders Outpatient Team.
The Substance Use Service team (SUS) is a multidisciplinary team made up of physicians, nurses, a mental health worker and peer support providers (individuals with lived experience). The team supports individuals presenting to the emergency department and those later being admitted to hospital who use substances. The team also provides follow up care for individuals who have discharged hospital and works alongside individuals to connect them to outpatient care; hospital and/or community-based health care services. The team operates from a harm reduction lens that allows patients the space and autonomy to make their own choices regarding both their substance use and their health choices. Services include assessment, consultation, shared care and capacity building across St. Joseph’s Healthcare Hamilton and community partners.
The primary responsibility of the Mental Health Worker will be to provide assessments and support to people accessing hospital. The Mental Health Worker works in collaboration with other members of the Substance Use Service (SUS) Team and the Concurrent Disorder Capacity Building Team and will provide individual and group treatment, with a particular focus on harm reduction and trauma informed care. The Mental Health Worker will also support in discharge planning as well as provide follow up care to individuals post hospital presentation and/or admission. As part of role, you will be working both in hospital and community which will include seeing patients in shelters, housing, encampments, and at community serving agencies. The Mental Health Worker also collaborates with other interprofessional staff and community partners to increase access to hospital and community-based care for those in medical need within the community. The Mental Health Worker maintains Standards of Practice as defined by their respective College.
QUALIFICATIONS
- Degree/Diploma in Nursing (RN) or Degree in Social Work (BSW or MSW) or Occupational Therapy OR current registration in good standing with the College of Psychotherapists with a Degree in a health-related discipline
- Current registration in good standing with a related regulated health professional College
- CPI-Crisis Prevention Training (or successful completion as part of clinical orientation) is required
- Current First Aid/CPR certification is required
- Minimum of five years’ experience in a harm reduction setting
- Demonstrated knowledge and experience working with people who actively use substances.
- Demonstrate advanced knowledge of individual counseling and group facilitation skills specifically related to harm reduction and trauma informed care
- Valid driver’s license and reliable transportation to be used in course of employment
- Minimum of 5 years recent related clinical experience in concurrent disorders/Addictions
- Demonstrated knowledge of individual counseling and group skills (ie. DBT, MI, IPT, CBT, ACT)
- Knowledge of risk assessment and management, including knowledge of trauma informed care and alternatives to seclusion and restraint, and associated risks.
- Knowledge and practice of safe and secure environment for clients/families/teams by listening, discussing, exploring, clarifying and acting on their concerns, needs and priorities.
- Proficient computer skills including successful completion of Resident Assessment Instrument – Mental Health (RAI-MH) training upon hire
- Demonstrated knowledge and skill related to crisis intervention and conflict resolution.
- Demonstrated leadership, superior problem solving, critical thinking skills and strong team building skills.
- Demonstrated interpersonal and de-escalation skills.
- Good work performance and attendance record is essential
Harm Reduction-Specific Qualifications
- Completion of a certificate program in addictions/concurrent disorders from an accredited educational institution preferred
- Minimum of five years’ experience in a harm reduction setting
- Demonstrated knowledge of the stages of change model and ability to utilize stage specific interventions
- Demonstrated competence in the facilitation of harm reduction, addictions/concurrent disorder groups (relapse prevention)
- Demonstration experience working with a harm reduction model including the provision of Naloxone
- Demonstrated competence in conducting the screening, assessments of patients with addiction and concurrent disorders including the identification of risk factors and symptoms of withdrawal
- Ability to provide clinical consultation through the demonstration of therapeutic techniques and interventions specific to concurrent disorders/addictions
- Demonstrated skill in the incorporation of evidence based practice and relevant referrals into
- Demonstrated ability to provide clinical support to patients with complex addiction/medical issues transitioning from hospital to community
- Demonstrated knowledge of and ability to access formal and informal community services and resources
- Demonstrated track record of providing leadership in building the organization’s capacity for delivering a hospital wide evidence-based concurrent disorder strategy
- Experience in leading case formulation, case conferences and providing clinical supervision
- Experience in applying empirical evidence and conducting research and evaluation
