Nursing Home Without Walls (NHWW) helps nursing and long-term care (LTC) homes support older adults to age where they call home with access to essential services and support. By helping older adults stay in their homes longer, NHWW supports their independence and reduces the need for early moves to long-term care.
Summary
Nursing Home Without Walls, in partnership with the Kiwanis Nursing Home, is looking for a highly motivated individual to fill the part-time role of Navigator. The successful candidate will play a key role in assisting seniors in the community to navigate various systems and resources, ensuring they can continue to live independently in their homes.
Responsibilities
- Assist clients in navigating systems such as Social Support NB to access necessary services
- Help seniors with applications for housing, financial benefits, and other essential resources
- Provide empathetic, non-judgmental support, empowering clients to make informed decisions
- Develop collaborative partnerships and actively engage in community networking
- Provide in-home assessments to determine client's needs
- Travel for meetings and training as required
Requirements
- Experience working with seniors, community development, or related field
- Valid driver's license and reliable transportation required
- Excellent verbal and written communication skills
- Strong organizational, administrative, and computer skills (proficient in MS Office)
- Ability to maintain confidentiality and safeguard sensitive information
- Ability to exercise strong judgement and instincts in supporting vulnerable populations when understanding and addressing their needs
- Capable of working independently with minimal supervision
- Successful completion of Vulnerable Sector Check and Medical Check
