As a Nurse Case Manager with Care At Home Services, you play a direct role in the lives of our clients each day by providing and promoting an environment to age gracefully in-place. You will partner with the Senior Operations Manager and Director of Operations to establish and maintain therapeutic environments and routines for our clients.
Responsibilities:
- Collaborate with family members to establish and maintain a therapeutic environment and routine for our clients.
- Supervise care and provide on-site training/care plan reviews to employees assigned to individual clients.
- Providing case management services for a variety of clients.
- Conducting staff meetings, reviewing staff performance, and conducting annual reviews of staff.
- Maintaining complete documentation of all client activities and events.
- Participating in client counseling sessions and telephone communications.
- Developing plans for case management including client care, treatment planning, budgeting, and reporting.
- Marketing within the community to build clientele and maintain communication with clients/partners to build business relationships.
- Necessary supplies and equipment purchase, maintenance, and organization of documentation on file.

Pay: $30.00 per hour
Expected hours: 37.5 per week
