Duties
- Conduct assessments to determine clients' needs and develop individualized service plans.
- Provide counseling and support to clients facing various challenges.
- Coordinate with other service providers to offer comprehensive support to clients.
- Maintain accurate and up-to-date case notes and records.
- Monitor client progress and adjust service plans as needed.
- Advocate for clients to ensure they receive necessary services and resources.
- Participate in case conferences and team meetings.
Qualifications
- Bachelor's degree in Social Work, Psychology, or related field.
- Proven experience in case management or social work.
- Strong organizational skills with the ability to prioritize tasks effectively.
- Proficiency in Microsoft Outlook and other computerized systems for record-keeping.
- Knowledge of records management best practices.
- Excellent leadership skills to guide and support clients through challenges.
- Ability to access community resources and services for client support.
- Fluency in English, both written and verbal.
- Experience managing projects and coordinating services for clients.
- Strong negotiation skills to advocate for clients' needs effectively.

Pay: $18.00-$25.00 per hour
