Wigwamen Incorporated is seeking an innovative, self-motivated and highly organized individual to fill a 12-month full-time contract position as a Follow-up Support Worker. The Follow-up Support Worker will provide housing case management supports to Indigenous clients with moderate needs who have been homeless and recently housed. Flexibility to work some evenings and weekends will be required.
The preferred candidate will have:
- Knowledge of, and experience working with the urban Indigenous community
- Minimum two years’ experience in a counselling role
- Comprehensive knowledge of housing issues, advocacy and support services
- Familiarity with the “Housing First” approach
- Ability to effectively navigate and assess community resources and supports
- Excellent interpersonal, verbal communication, report writing and facilitation skills
- Computer literacy, including Word, Excel, internet and email applications
- Experience working in a small team, within vulnerable and marginalized sectors and with clients with mental health issues and/or addictions
- Working knowledge of an Indigenous and/or other language would be an asset
- A valid driver's license and access to a vehicle would be an asset
Responsibilities will include:
Client Support:
- Meet with Indigenous clients that are homeless or at risk of homelessness, conduct street outreach, and offer supportive counselling and crisis intervention supports·
- Conduct regular home visits; actively involve the client in planning, goal setting and decision-making
- Provide responsive, culturally relevant and client-centred services to help establish connections to community-based supports, services and programs
- Actively participate in eviction prevention activities, including liaising with landlords to ensure rent is paid, and if needed, facilitate conflict and landlord mediation to encourage housing stabilization
- Provide rehousing supports and assistance to clients where a tenancy cannot be sustained
- Assist clients in moving towards achieving long-term housing and income stability which may include advocating on behalf of clients with landlords, OW, ODSP, and other services
- Provide life skills training in areas including landlord and tenant rights and responsibilities, budgeting, housekeeping, pet care, addiction treatment, mental health care, ID and document replacement services, volunteer opportunities, job placement, and job training, etc.
Project Administration:
- Develop and monitor an individualized case management plan for clients that focuses on setting and monitoring goals (both short and long-term), including discharge plans, crisis plans and mental health addictions prevention plans (where applicable)
- Keep detailed client records as set out in the overall project plan; submit monthly reports in accordance with Program requirements
- Monitor the effectiveness of supports; recommend changes to the types of support offered
- Other job-related duties as required by Wigwamen Incorporated
