Duties & Responsibilities:
- Assist clients with housing related supports, such as, eviction prevention strategies, covering rent or utility arrears, assistance with first and last rent payments and exploring funding sources to attain rent subsidies to provide rent relief
- Implements case management with clients, which includes organizing their financial affairs, setting up pay-direct from social services, gathering documents required for the Indigenous Supportive Housing Program, developing individualized care plans, recommending interventions including referrals, supportive counselling, aftercare, and follow-up services for clients after they are housed
- Provide advocation and mediation between the client and their landlord and supporting the client if required to attend Landlord and Tenant board hearing
- Support clients residing in buildings with units allocated to Anishnawbe Health Toronto through established partnerships with housing providers
- Accompany clients to their Furniture Bank appointments
- Complete client intakes and referrals to other agency services
- Chart notes in the Electronic Medical Record database
Qualifications & Experience:
- Bachelor of Social Work or two years of equivalent, relevant work experience in a social service field; especially with homeless clients
- Required to learn about Indigenous culture, history, traditions, medicines and healing approaches
- Comprehensive knowledge of client-centered interventions and community resources experience with case management and counseling
- Proficiency with electronic charting software
- Strong interpersonal, advocacy, collaboration and conflict resolution skills and the ability to work independently and as a member of a multidisciplinary team
- Awareness of housing resources within the GTA (e.g. Employment and Social Services, Indigenous housing providers, subsidy rent supports, Landlord and Tenant Board, etc.)
- Valid Ontario driver’s license
