Certified Personal Support Worker (PSW) – Palliative Home Care
Overview
We are seeking a Certified Personal Support Worker (PSW) with a compassionate and caring nature to provide dedicated in-home palliative care. This important role supports individuals and their families during one of life’s most sensitive stages. The successful candidate will demonstrate emotional strength, empathy, and professionalism while delivering person-centered care focused on comfort, dignity, and quality of life.
Key Responsibilities
As a PSW providing palliative support in a home setting, your duties will include:
- Assisting the client with personal care activities such as bathing, grooming, dressing, toileting, and incontinence care, with gentleness and respect.
- Providing emotional support and compassionate companionship to the client and family members.
- Supporting mobility and repositioning to maximize comfort and prevent pressure injuries.
- Preparing light meals and assisting with feeding if required, based on the client’s needs and preferences.
- Assisting with medication reminders or administration (as per your certification scope), and closely monitoring for any changes in condition.
- Communicating with family members and collaborating with nurses, doctors, and palliative care teams to ensure continuity and quality of care.
- Supporting the client and family during end-of-life moments, offering presence, sensitivity, and calm reassurance.
- Maintaining a clean, safe, and peaceful environment appropriate for home-based palliative care.
- Documenting care provided and reporting any concerns or updates to supervising healthcare professionals.
Qualifications & Experience
We are looking for candidates with:
- A valid Personal Support Worker (PSW) certification from a recognized training institution.
- Previous experience in palliative or end-of-life care, ideally in a home or hospice setting.
- A warm, compassionate, and caring approach to caregiving.
- Emotional resilience and the ability to support individuals and families during grief and loss.
- Knowledge of palliative symptoms such as pain, shortness of breath, and fatigue, and how to support comfort measures.
- The ability to maintain clear, respectful communication and uphold the dignity and privacy of clients at all times.
- Experience with basic household tasks to help maintain a safe and supportive home environment.
