The John Howard Society of PEI is anticipating the need for a passionate and motivated caseworker to join our team. As a caseworker, you will play a vital role in providing services and case management to individuals and families who are at risk of or experiencing homelessness. Key Duties/ Responsibilities:
- Reports directly to the Wrap Around Housing Program Manager
- Support individuals to find and secure housing through case management and connecting them to suitable resources
- Support clients through advocacy and service navigation
- Assess client needs based on based on bio/psycho/social requirements using the Service Prioritization Decision Assistance Tool and internal assessment tools
- Create goal-oriented treatment plans for clients that are reflective of their needs
- Utilize a strength-based, trauma-informed approach promoting life stabilization
- Accept referrals from a range of sources including self-referrals, community-based organizations and Government organizations
- Network with various community and Government-based organizations
- Take a collaborative, client-centered approach
- Participate and lead case conferences with other community agencies
- Complete and maintain documentation and casenotes in the HIFIS database
- Be prepared to work in a range of settings including the client’s home and community settings such as food banks, shelters, hospitals, corrections, etc.
- Support data collection and reporting for the Wrap-Around Program
- Other duties may be required
Key Skills Required:
- A thorough understanding of housing and homelessness based on the Housing First Model
- The ability to work independently as well as in a team environment
- The ability to organize, prioritize and multitask
- The ability to conduct assessments with clients
- The ability to evaluate risk
- The ability to problem-solve creatively
- The ability to work well under pressure while maintaining a positive attitude
- Strong communication skills, both written and oral
- Possess a working knowledge of community resources and how to access them
- Proficient with technology (electronic databases, computer programs and software, cloud-based drives)
- Familiarity with local, Provincial, and Federal resources an asset
Excellent interpersonal skills
- Demonstrable initiative and drive to seek resources and implement change
- Represent/promote the organization in a positive manner within the community
Qualifications:
- University degree in human services and/or post-secondary education and certification in the field of human services or an equivalent combination of education and experience
- At least 2 years experience working in client-facing services (healthcare or social work)
- Experience working with marginalized populations (including but not limited to individuals experiencing homelessness, individuals who use substances, individuals with mental health issues)
- ASSIST training would be an asset
- CPR- First aid certified would be preferred
- Ability to advocate effectively on behalf of the clients
- Understand and practice professional boundaries
- A Criminal record check is required
- Must have a valid Prince Edward Island driver’s license and have access to reliable, insured transportation.
- Must be willing to commute to client and organization locations outside the office.
- Must be legally eligible to work in Canada.
