Big Hearts Home Care is seeking an experienced RN to join our healthcare management team. We value compassion, integrity, innovation and excellence in care. As a Care Manager you will be supporting our operations in the Lower Mainland, managing service to the elderly, injured workers and accident victims. You will be responsible for ensuring our clients receive quality home care and healthcare services as we enrich the lives of clients within our community.
Duties and Responsibilities
The Case Manager will be responsible for the following:
- Meet and communicate with new potential clients and family members
- Manage a team of passionate caregivers
- Design and implement care plans
- Liaise with hospital, WorkSafe BC staff and community health workers
- Provide one-on-one training to caregivers
- Develop strong referral relationships in the community
- Develop and implement best business practices in collaboration with our team
Position Requirements
- 5+ years’ experience in the healthcare field
- Worksafe Experience (required)
- Registered Nurse (RN license) with at least 3 years’ experience or equivalent experience.
- Valid driver’s license, access to a vehicle and comfortable driving throughout Vancouver
- Comfortable using various healthcare systems, MS Office, Adobe, and email
- Available for odd shifts in the evenings, early mornings and weekends
- Self-managed as well as love working in a team on fulfilling projects
- Love bringing a positive customer service experience to clients
