Qualifications:
- A Diploma from an accredited College in a Health or Social Service Program, with an addiction concentration or complimented by other recognized/relevant Addiction Studies required
- Valid certification in CPR, First Aid (have taken it once), and Crisis Intervention preferred or required within three months of start
- Valid Ontario unrestricted Class ‘G’ Driver’s license required
- Ability to be deployed by vehicle, bicycle or on foot, working in teams of two
- Within 2 years of hire- will obtain an Addictions and Mental Health Ontario Graduate Certificate
Principal Duties:
- Provision of withdrawal management care to clients in the Integrated Withdrawal Management Program (IWMP)
- Crisis intervention and support for clients
- Perform health and wellness checks, provision of warming, cooling and harm reduction supplies
- Screening, assessment, monitoring, provision of physical care, providing education and treatment options to the clients
- Formulation and documentation of a comprehensive care plan and discharge plan in collaboration with the client
- Facilitates individual and group counseling sessions, crisis intervention and aftercare support services
- Outreach services and administrative duties as required
- Referral to other services, for example: primary care, housing and other social supports
- Participates in unit/department and Hospital Quality Improvement initiatives
- Completes continuing education requirements as necessary
- Other related duties as assigned
