Job Summary
At the heart of our work is an interdisciplinary approach that delivers holistic, community-based care – with a strong focus on individuals who experience barriers to accessing health services. Our team is deeply committed to inclusivity and works within the social determinants of health framework, emphasizing prevention, health promotion, and client education.
In partnership with the West Toronto Ontario Health Team (WTOHT), LAMP Community Health Centre is seeking passionate and community-driven individuals to serve as Preventative Care Ambassadors for the Locally Driven Population Health Models (LDPHM) initiative.
Reporting to the Project Manager, the Preventative Care Ambassador supports health equity and wellness initiatives across West Toronto and Peel. This role involves supporting the implementation of health action care plans, facilitating health promotion activities, supporting targeted programs, and providing system navigation support. The Ambassador works closely with the Community Heath Workers and interdisciplinary teams to engage diverse communities through inclusive, culturally responsive outreach aligned with LAMP’s mission of delivering equitable and quality healthcare.
Primary Responsibilities
- Facilitate health promotion activities and community events
- Support the delivery of targeted programs and services
- Coordinate the implementation of care plans in collaboration with Prevention Care Team Community Health Workers
- Provide navigation support across the healthcare system
- Identify and address barriers to care access
- Foster inclusive, culturally responsive engagement with diverse populations
Required Qualifications
- Post-secondary degree in health sciences, social work, or a related field applicable to community health/community work
- 1–2 years of experience coordinating community-based health programs, events, or workshops
- Strong communication skills (written and verbal), with preference for candidates fluent in Spanish, Russian, Polish, Tibetan, or Mandarin
- Proven ability to collaborate with external partners and multidisciplinary teams on shared initiatives
- Solid understanding of preventative care principles and barriers to healthcare access among marginalized populations
- Strong organizational, problem-solving, and critical thinking skills to support healthcare navigation and program development.
