The Opportunity!
Under the direction of the Manager, Passport Program, the Passport Outreach Coordinator (POC) works positively and proactively in the community with clients and their families/supporters who need additional assistance to develop and implement plans and budgets to use their Passport funding. The POC is an experienced professional, with solid knowledge of developmental services in Toronto, skilled in developing relationships with families and natural support networks and understands individualized approaches to supports and funding. The POC carries out responsibilities in accordance with FST policies and practices.
- Manages a caseload of Passport clients as assigned by the manager.
- Reaches out to clients who have challenges using their Passport funds.
- Provides client service in the community (e.g., home visits, community meetings).
- Works with clients to develop a goal-oriented plan and budget to use Passport funding using an individualized and person-directed approach; focusing and building on the client’s strengths, and addressing identified needs.
- Supports clients/families to locate, access, and evaluate appropriate supports and community resources (e.g., personal support workers, day programs, residential options).
- Advocates for clients and provides information, referral, and support, as required
- Strengthens the capacities of clients, families and communities by fostering their inclusion, self-development, and participation within communities.
- Works with communities and members of the public at large to encourage and advocate for support individuals with disabilities by valuing and respecting their strengths, competencies, expertise, resilience, and capacities.
- Monitors client budgets and ensures that clients use funds within the Passport guidelines.
- Ensures that client information in the database is current and accurate including funding amount.
- Receives phone calls and emails and provides appropriate information.
- Ensures that client files are current and completed as per agency policies and procedures.
- Provides informed referrals to clients and remains up-to-date on community programs and services.
- Represents and promotes the interests of clients, the Passport Program, and FST with external groups and community as required.
- Completes other administrative tasks as required in a time sensitive manner.
- Collaborates with colleagues to meet clients’ needs and ensure quality service.
- Participates in regular supervision meetings with the manager.
- Participates in professional development as relevant for their position.
- May be assigned additional duties consistent with this role.
- Adheres to all safety protocols of the organization.
Qualifications
- A post-secondary degree in social work or a diploma as a social service worker or an equivalent combination of education and experience over a minimum three-year period (foreign credentials and relevant experience will be considered).
- Minimum of three years of experience in the social services field.
- Ability to promptly and effectively address potential risk(s) involving clients’ well-being and safety.
- Sound understanding of issues faced by people with developmental disabilities.
- Demonstrated experience working in a person-directed approach.
- Experience supporting individuals to identify community inclusion goals and work towards their achievement.
- Experience assisting clients to develop budgets and plans.
- Extensive knowledge of the developmental services sector and of community-based resources in the Greater Toronto Area. Detail oriented
- - able to work with numbers and budgets as well as accomplish other administrative tasks in a fast-paced environment with tight deadlines.
- Good organizational, time management and computer skills including familiarity with database applications.
- Strong interpersonal and communication skills (e.g., verbal, written and listening).
- Ability to work independently and apply creative problem solving strategies.
- Ability to work collaboratively and to develop strong working relationships with other services providers, internally and externally.
- Demonstrated commitment to principles of social justice, equity, inclusion and cultural competency.
- Ability to work flexible hours (e.g., one evening per week).
- Valid Ontario driver’s license and daily access to a vehicle in order to provide client service in the community (e.g., home visits, community meetings).
