Overview
The Community Rehabilitation Worker is responsible for providing support to individuals in the residential, vocational, or community settings. The Community Rehabilitation Worker will participate in communicating with individuals, Key Workers, Job Coaches, Team Leads, other employees and parents/guardians. The Community Rehabilitation Worker will record data in the individual's programs.
Position Duties and Responsibilities
90% Direct Individual Support
- Maintaining professional standards of confidentiality
- Always showing respect for the individual; presenting the individual in the most positive and objective manner.
- Following through with instructions on goals and objectives in the individuals program plans.
- Assisting in community participation.
- Developing dialogue with individual, guardian, parents, and employees in addressing needs and issues.
- Following through with individual's budgeting program.
- Ensuring the environment and equipment is safe for the individual
- Assisting the individual with appropriate medications.
- Initiating community integration.
10% Record Keeping
Under the mandate and policies of this agency, the Community Rehabilitation Worker will maintain a system of documentation and record-keeping as follows:
- Completing and filling out incident reports and records as required by policy.
- Complete medication recording sheets for each instance of dispensing and administering medication as per policy.
- Maintaining data collecting as set out by the Individual Program Plan.
- Recording daily logs and communicating issues regarding the individual.
Requirements
- Standard First Aid that includes CPR/AED- within three months of employment.
- A current criminal record check with vulnerable sectors.
- A vehicle and valid drivers’ licence is required for this position.
- Strong interpersonal skills with the ability to build rapport with individuals from diverse backgrounds.
- Excellent communication skills, both verbal and written.
- Ability to work independently as well as part of a multidisciplinary team.
- Experience in rehabilitation settings or similar environments is a plus.
- Compassionate and patient demeanor with a genuine desire to help others succeed
- Demonstrate an ability and willingness to give and receive honest, balanced feedback
- Willingness to undergo any required training or certification relevant to the role.
