The Role: Housing Support Worker (Case Manager- YTA)
The Case Manager works within Housing First programs for youth ages 16 to 24, who are currently experiencing, or are at risk of homelessness. With a focus on harm reduction, this scattered-site housing program aims to house youth in the community of their choice. Some of the barriers Case Managers reduce for their clients include those to housing, basic needs, medical appointments, advocating for system supports, mental health services and addiction counseling. Case Managers have a caseload ranging from 1 to 8 youths at a time.

Key Tasks & Responsibilities:
  • Provide individual coaching and support to youth who are at risk of being homeless and/or sexually exploited.
  • Support youth by advocating for supports and placements needed to effectively deal with their issues surrounding homeliness and/or sexual exploitation.
  • Providing assertive outreach services to identify and support youth that are at-risk of homelessness and being sexually exploited.
  • Intervening and supporting youth through crisis situations as they occur.
  • Help a youth transition to heathier lifestyles by providing 1 to 1 support to accessing community services and natural supports.
  • Acting as a liaison as needed between families, social workers, school representatives, doctors and other professionals involved with the youth.
  • Assist youth in developing and implementing goal plans.
  • Work with youth on building life skills to live successfully within the community.
  • Provide referrals and support access to housing, food, clothing, hygiene, and other necessities.
  • Assisting and connecting youth with health care supports
  • Complete a variety of assessments with youth and documentation surrounding casework, goal planning and Critical Incident Reporting.
  • Assist youth in the community with resumes, vocational skills, and obtaining educational resources.
  • Other duties as required.
Knowledge and Skill Expectations
  • A post-secondary degree or diploma in the social sciences or human services field. Equivalent combination of education, experience and training will be considered.
  • Minimum 2 years of direct work experience with vulnerable populations.
  • Able to work flexible hours including the occasional weekends, events, and holiday coverage.
  • Valid Class 5 Drivers License and Drivers Abstract.
  • Reliable vehicle as this position requires you to transport youth.
  • Knowledge of Foyer Model’s Philosophy and Operations, Housing First for Youth, and Harm Reduction Models.
  • Excellent communication and collaboration skills.
  • Able to provide a clear Criminal Record Check with vulnerable sector search and Child Intervention Record Check
Reporting Relationship
This position reports directly to a Trellis Program Manager.
