About the Role
In alignment with the Mission, Vision, Values, and strategic directions of Providence Health Care, the Social Worker promotes a safe, respectful, and inclusive environment for patients, families, and staff. You will work collaboratively within an interdisciplinary team under the direction of the Patient Care Manager, with clinical guidance from the Social Work Site Leader, Professional Practice Leader, and Research, Education and Practice Coordinator.
Key Responsibilities
- Deliver therapeutic clinical counseling to individuals with complex substance use and mental health needs.
- Complete comprehensive biopsychosocial assessments to inform treatment planning.
- Act as a leader in discharge planning, collaborating with care teams, families, and community partners.
- Offer consultative services on community resources and access pathways for clients and families.
- Coordinate and lead clinical/case conferences and in-service education sessions for staff.
- Actively participate in committees, working groups, and quality improvement initiatives.
- Maintain up-to-date clinical documentation and stay informed on professional developments and best practices in Social Work.

Qualifications:
- Master’s in Social Work from an accredited university
- One year of recent related experience in a designated program area, or an equivalent combination of education, training, and experience
- Current full registration with the British Columbia College of Social Workers
- Bachelor of Social Work degree from an accredited School of Social Work, with an equivalent combination of education, training and experience may be considered. Current full registration with the British Columbia College of Social Workers
Skills and Abilities
- Demonstrated ability to communicate effectively with patients/residents, families, staff, physicians, and other healthcare providers
- Knowledge of legislation and government policies related to healthcare
- Ability to gather relevant data and conduct biopsychosocial assessments
- Experience in conducting patient/resident assessments
- Knowledge of other healthcare disciplines and their roles in patient/resident care
- Ability to draw out information to understand the patient's/resident's situation and explore issues beyond initial concerns
- Proven ability to work collaboratively as a member of an interdisciplinary team
- Skill in conflict resolution, including guiding others through resolution issues
- Physical ability to perform the duties of the position
- Strong independent work skills, with the ability to prioritize tasks and workload
- Proficient in operating related equipment
- Willingness and ability to participate in research
- Experience in providing supervision and teaching to students
- Ability to facilitate and influence group and inter-group meetings, processes, and decision-making

What You'll Do – Day to Day Responsibilities
As a Social Worker in the Road to Recovery Program, you will play a key role in supporting clients across their journey through substance use and mental health recovery. Your work will span from assessment and counselling to advocacy, discharge planning, education, and collaboration with interdisciplinary teams and community partners.
Clinical Practice & Direct Support
- Provide therapeutic clinical counselling to patients, families, and groups, helping to identify treatment goals, coping strategies, and facilitate behavioral, emotional, and relational change.
- Conduct comprehensive biopsychosocial assessments, gathering relevant information to support care planning based on patients’ social, cultural, financial, and environmental contexts.
- Participate in interdisciplinary care planning, ensuring client-centered discharge and transition strategies are in place, particularly for individuals with complex needs.
- Offer consultative support to clients and staff on accessing community and financial resources to facilitate continuity of care.
Documentation & Evaluation
- Maintain detailed and accurate clinical records, including progress notes, assessments, discharge planning, and workload data, in alignment with regulatory and organizational standards.
- Support ongoing program evaluation by contributing insights and recommendations to improve clinical care and service delivery.
Leadership & Advocacy
- Facilitate understanding and application of relevant legislation including the Mental Health Act, Adult Guardianship Act, and Health Care Consent and Care Facility Act.
- Act as a Designated Responder, Incapability Assessor, or Rights Advisor as needed.
- Participate in care conferences, case reviews, and interdisciplinary meetings to advocate for client needs and integrate social work perspectives into planning.
Education, Training & Research
- Develop and deliver in-service education and training to interdisciplinary teams on social work-related topics.
- Support and supervise social work students, providing guidance and clinical learning opportunities.
- Engage in ongoing professional development to stay current with best practices in trauma-informed care, harm reduction, and substance use recovery.
- Participate in research initiatives and special projects, including protocol development, literature review, grant writing, data collection, and dissemination of findings.
Collaboration & Systems-Level Involvement
- Contribute to hospital and community-based committees, helping to identify service gaps and improve program efficiency.
- Facilitate and participate in group programming, offering psychoeducation and therapeutic support.
