Runnymede LTC Home, a 200-bed long-term care home in Toronto, Ontario is an exciting new project that will be dedicated to enriching the lives of our future residents in partnership with UniversalCare, our family members, valued employees and community partners. Residents are expecting to be admitted in August 2025.
Reporting to the Administrator, the Social Work Manager position is a critical role that works in collaboration with an interdisciplinary team to advocate and provide support and services to residents and family members.
As part of our team, your role as a Social Work Manager contributes to our resident experiences in the following ways:
- Planning, developing, implementing and evaluating individual social work services to meet the identified needs of residents and family members; ensuring those activities are aligned to the home’s strategic plan, mission and values.
- Promotes a resident-centered care approach that engages and supports residents and family members with respect to living in the home, behavioural and life adjustment concerns, etc.
- Acts as an advocate for the resident and promotes the Resident Bill of Rights.
- Maintains a records system in accordance with Company policy and legislative requirements.
- Participates in resident multi-disciplinary care conferences and meetings, as required.
- Establishes and maintains communications with other disciplines, residents and families to ensure individual resident needs are being met.
- Collaborates and supports residents and family members with respect to the provision of external community services.
- Completes RAI MDS as required.
- Works in accordance with the organization's Health and Safety Policies and Procedures and in compliance with the Occupational Health and Safety Act.
- Performs other duties as assigned.
Qualifications:
· Degree in Social Work from a recognized educational institution.
· Must have a minimum of one (1) year experience in a health-care setting, preferably long-term care.
· Current membership in the Ontario College of Social Workers and Social Service Workers or eligible for membership with the Ontario Association of Social Workers or the Ontario College of Social Work.
· Must posses excellent written and oral communication skills and relationship-building abilities.
· Proficiency with Microsoft Office and other computerized documentation systems.
· Possess specialized knowledge in Gerontology, Behavioural Disorders, Complex and Palliative Care.
· All applicants must successfully pass the prescribed Vulnerable Sector check.
· Have a passion to promote person-centered care and work with the senior population.
