As a Rehabilitation Assistant with VCH you will:
- Function under the operational supervision of the Manager or Team Leader and the work direction of a licensed designated occupational therapist or physiotherapist to work as a member of the health care team.
- Provide assigned physical / occupational therapy therapeutic interventions.
- Assist in modification of OT/ PT care plans and facilitate transition to community programs in order to enable clients to maximize potential for independence.
- Provide functional direction to family, report relevant observations of client's condition in writing or by utilizing basic computer literacy.
- Manage equipment safety and inventory.
Home Health programs provide a range of health care and support services to people living in the community who have acute, chronic, palliative or rehabilitative health care needs.
The Client Population:
Home Health clients typically have on-going health issues and/or complex personal and/or social issues, requiring care planning and coordination of appropriate resources. Clients include frail elderly, younger clients with disabilities, and adults with chronic illnesses requiring supports to maintain/promote independence. Raven Song serves the midtown area including Olympic Village, Mount Pleasant, Main Street and Riley Park neighbourhoods.
Education & Experience
- Completion of a recognized Diploma level Rehabilitation Assistant Program with Occupational and Physical therapy components plus one (1) year's recent, related experience working with target population.
- Valid BC Drivers License. Local area travel requires the use of a personal vehicle for which mileage will be reimbursed.

- Ability to establish and maintain rapport with clients.
- Ability to motivate clients and to encourage progression through therapeutic goals.
- Demonstrated ability to provide self-care instruction and education.
As per Ministry of Health policy, all health care workers working in publicly-funded health care facilities are required to report their past receipt of certain vaccines or history of certain infections. Collecting these records will allow for offering of any missing vaccines, and for appropriate actions to be taken in the event of any future exposure to a communicable disease or during outbreaks. For all new hires and appointments to Vancouver Coastal Health, you will be asked to provide this information as part of the onboarding process.
