Cardiac Care Network Team

Reporting to Manager, Cardiac Care, the Regional Cardiac Care Coordinator (RCCC) will facilitate timely access to cardiac services identified under the Provincial Wait Time Strategy and funded as a priority program by Ontario Health (OH) following the Best Practice Guidelines established by CorHealth-OH. The role requires the employee to liaise and provide ongoing communication with referring physicians and centres throughout Ontario, and track patient wait times and stability while they await the procedures. The RCCC work collaboratively with the clinical and administrative teams to schedule procedures in alignment with programmatic guidelines. The RCCC also provides patients and families with education and support.

Regular Part-Time

Hours of work: Up to 22 hours per week (no guarantee of hours)

Rate of pay: $52.16 per hour - $65.20 per hour

Qualifications

  • Undergraduate degree in Health or Social Studies related field
  • Current certificate of registration from a college recognized under the Regulated Health Professions Act
  • Current Advanced Cardiac Life Support (ACLS) certification preferred
  • Minimum five (5) years recent experience in an advanced Cardiac Care Program at a clinical level
  • One (1) to three (3) years recent, related experience in hospital administration preferred
  • Advanced clinical assessment skills; ideally in cardiology or cardiac care
  • Strong written and verbal communication skills
  • Strong analytical and problem-solving skills
  • Proficiency in patient booking and scheduling systems
  • Advanced computer skills with Microsoft Office (Word, Excel, FileMaker Pro and Access)
  • Demonstrated practice and commitment to the principles of patient and family centered care
  • Demonstrated practice and commitment to patient and staff safety at LHSC
  • Demonstrated practice and commitment to LHSC's vision, purpose and values
  • Demonstrated ability to attend work on a regular basis
