Position Summary :
The Regional Cardiac Care Coordinator (RCCC) triages referrals for patients awaiting advanced cardiac procedures within the Cardiac Program. The RCCC facilitates review with cardiac physicians to provide appropriate and timely access to care. The RCCC communicates with physicians caring for patients awaiting cardiac procedures to provide direction and bridge care between referring sites and the Regional Cardiac Care Program. They are responsible for maintaining the waitlist for each procedure and monitoring the stability of patients while awaiting cardiac procedures. The RCCC ensures patients receive education about upcoming procedures. Patients and families are able to contact the RCCC for support while they wait. The RCCC ensures maintenance and accuracy of the provincial database for all patients awaiting advance cardiac procedures in Ontario.
Responsibilities:
- Triage regional referrals to facilitate timely access to cardiac services, review clinical information, communicate with referring and procedural physicians, prioritize urgency, plan daily procedural operations of the program
- Communicate with patients and family members to provide support and education while awaiting cardiac procedures
- Assist in the development, implementation and/or revision of program goals and program improvement projects. Participate in Quality and Operations committee meetings. Participate in additional meetings as required.
- Collect patient data, maintain provincial Cardiac database, review and report on procedural volumes, prepare monthly statistical data which helps guide program planning, participate in Ontario Health projects as required
- Adapt communication and interpersonal skills to deal effectively with patients and their families, co-workers, physicians, volunteers and others within our health care community.

- Effectively apply organizational and time management skills to prioritize workload, to problem solve, and to adapt to frequent interruptions and change.
Position Requirements:
- Registered Nurse in good standing with the College of Nurses of Ontario
- BScN or equivalent
- Three (3) to five (5) years clinical cardiac experience in an Acute Care Hospital
- Strong planning, organizational, leadership and prioritization skills
- Demonstrated critical thinking and problem solving skills
- Proven ability to be extremely self-directed and self-motivated
- Demonstrated ability to work independently and in a team environment
- Committed to providing high quality patient-centred care
- Demonstrated effective written and verbal communication skills
- Proficient computer skills including Cerner, Microsoft Word, Microsoft Excel
- Successfully able to perform their function in accordance with prescribed standards that focuses on the priority of People Safety and the delivery of the highest standard of Quality Care.
- Demonstrate the physical and mental capabilities necessary for proper and efficient performance of the essential duties of the position.
