NorWest Community Health Centres is a community-based, non-profit organization funded by Ontario Health, committed to providing integrated primary care and health promotion services for individuals facing barriers to care. We work collaboratively to ensure equitable, culturally safe, and accessible care across all programs and services.

We are currently seeking a contract Project Coordinator – Inter Professional Care Team (IPCT). As a key member of the NorWest Community Health Centres - Inter Professional Care Team, the Coordinator will support the planning, delivery, and evaluation of interdisciplinary care programs that are equity-responsive and trauma-informed. This role focuses on improving access and coordination of services for racialized, aging, underhoused, populations living within Social Housing Unit throughout the City of Thunder Bay. The Coordinator will lead community engagement activities, facilitate internal and external partnerships, and support the implementation of innovative care models across multiple settings.

This is a newly funded position. The position offers a competitive salary, excellent group health benefits and option to join the Healthcare of Ontario Pension Plan (HOOPP).
QUALIFICATIONS:
• Post-secondary education in Health Sciences, Community Development, Social Work, Public Health, or related discipline.
• Minimum three years’ experience in interdisciplinary health service coordination or program delivery.
• Demonstrated experience supporting racialized or marginalized communities, including Black/Indigenous/newcomers/none insured/2SLGBTQIA+ and aging populations.
• Strong knowledge of the social determinants of health and culturally responsive care models are an asset.
• Exceptional project coordination, communication, and relationship-building skills.
• Experience with program planning, implementation, and evaluation.
• Proficiency with Microsoft Office, electronic health records, and data tracking systems.
• Valid Ontario driver’s license and access to a reliable vehicle for community engagement activities.
