POSITION SUMMARY:

The Patient Navigator provides active leadership to facilitate and coordinate patient flow from admission to discharge, across acute and post-acute care areas in partnership with stakeholders. Using a collaborative approach and advanced communication skills, the Patient Navigator builds strong relationships with patients, families, care team members and system partners. The Patient Navigator plays a key role as facilitator, communicator and change leader in support of both the daily management of patient flow (bed flow management, repatriations, eliminating barriers to specific flow challenges) and the improvement of processes supporting access, quality and safety.

Specific processes within patient navigator scope can include the following:

  • Facilitate the bed management process, organizational flow (from admission to discharge), and problem-solving, in partnership with stakeholders.
  • Coordinate repatriations to and from appropriate programs and services at JBH.
  • Attend ED and inpatient unit rounds to determine and anticipate unit and site-wide demand and supply for beds
  • Support the Integrated Comprehensive Care program (including patient screening, follow up phone calls and on call responsibilities).
  • Process and provide oversight of escalations for patients requiring an Alternative Level of Care (ALC).
  • Participate in programs to support patient flow such as Early Supportive Discharge (ESD) and remote patient monitoring.
  • Participate in related organizational committees and initiatives; support with data collections and interpretation
  • Coach staff to improve daily processes and decision making to align to policies/procedures.

To support process and system level improvement, the Patient Navigator contributes to and implements policies and procedures related to the care delivery process and patient flow strategies, assists in the development of quality and process improvement initiatives, participates in, and shares responsibility for, ensuring that performance is optimized, from the unit level, to the corporate level.

The ideal candidate will have excellent interpersonal and communication skills combined with the ability to collaborate within and across professions; proven facilitation and conflict resolution skills; demonstrated ability to guide and influence others; experience with performance improvement knowledge and skills, preferably with experience in LEAN; ability to prioritize and manage resources, comprehensive knowledge of and experience with acute care hospital patient care processes. Formal leadership experience is an asset. JBH is interested in professionals who thrive on team participation and are committed to providing the best care possible for our community.

QUALIFICATIONS:

  • A Healthcare Professional in good standing in the Province of Ontario;
  • A Bachelor’s degree preferred in a healthcare discipline or related field (Nursing, Occupational Therapy/Physiotherapy, Social Work, etc).
  • Understanding and familiarity with patient flow and supporting policies and procedures, and have comprehensive knowledge of relevant legislation and community resources.
  • Minimum of five (5) years of experience working in an Acute Care/Emergency Department setting.

In addition, the successful candidate will have:

  • Outstanding communication skills and the proven ability to communicate professionally with stakeholders, balancing competing priorities.
  • Demonstrated ability to provide support and leadership to advance evidence based inter-professional patient centered care.
  • Demonstrated ability to deal with ambiguity in a rapidly changing environment.
  • Strong problem-solving, organizational and time-management skills.
  • Demonstrated ability to work as an effective member of a multidisciplinary team.
