Collingwood General and Marine Hospital (CGMH) is an 84 bed hospital located in Collingwood, Ontario (approximately 90 minutes north of Toronto) that serves more than 74,000 permanent residents and 3.5 Million annual visitors to the communities of Wasaga Beach, Collingwood, Clearview and the Blue Mountains. CGMH is an acute care hospital providing emergency care, diagnostic services including lab, imaging and cardio respiratory therapy, as well as two inpatient units (medicine and surgery). In addition, CGMH also provides care in specialty areas including obstetrics, orthopaedics, intensive care and surgery. The Hospital also provides out patient care including dialysis and a wide range of clinics including mental health and rehabilitation services. CGMH continues to provide care close to home for our community and plays a key role as an integrated orthopaedic centre for our region. The Opportunity:
The Patient Flow Navigator will provide consultation for patients with complex needs in planning for discharge while supporting the patient experience and the patient plan of care. The Patient Flow Navigator provides admission avoidance support to the ED and/or inpatient units to reduce hospital length of stays and repeat admissions. The Patient Flow Navigator will work collaboratively with patients, families and the care team to identify possible barriers to return to home and to develop a safe and supportive discharge plan. Priority will be given to patients who are experiencing increasing frailty and are high risk for medical and psychosocial issues.
Qualifications:
- University, College or Diploma in Health related profession or equivalent
- Current registration with any regulated health care professional college in ON
- Ability to organize patient care demands and set priorities while working independently and collaboratively with all members of the health care team including the patients and families
- Demonstrated assessment, health promotion, health teaching skills, critical thinking, problem solving and decision making skills
- Excellent communication, organizational and interpersonal skills with the ability to perform duties in a professional and courteous manner
- Excellent customer service demeanour
- Physically able to perform the essential duties of the role

- Committed to professional growth & development
- Committed to maintaining confidentiality
- Education in gerontology and/or chronic disease management an asset
- Familiarity with Meditech is an asset

- Must have an ability to effectively communicate (written and verbal) in the English language in order to provide excellent communication with team members, patients and families. Ability to communicate in additional language(s) is considered an asset.

- Must be able to perform all bona fide and essential duties of the position, which may include physical demands such as moving/transferring patients, pushing, pulling, bending, etc.

- Experience working with discharge planning/case & bed flow management in an acute care setting is preferred
Duties and Responsibilities:
- Provide consultation to identify barriers to return to home of ‘patients in the ED Department and on the inpatient unit to initiate a safe discharge plan on admission in order to reduce admission LOS.
- Support ‘Right Care-Right Place’ providing transitional care to patient and families while facilitating patient flow processes including surge management

Foster the ‘Home First’ philosophy and system/regional initiatives
Work with our internal Allied Healthcare Professionals and Community Agencies to provide appropriate supports in the community to ensure safe and timely patient transitions

Hours of Work: Days (8:00 to 16:00) including weekends. Please be reminded that as a Hospital, in order to provide the necessary patient care, our operations run 24/7. Therefore, depending on the program for which you are applying, it may be an expectation that you are available to work week days and weekends, as well as all shifts - days, evenings, and nights.
Regularly scheduled for up to 24 hours per week (48/pay).
Wage: $44.79 - $53.19 per hour
