Mobile Health Unit Coordinator
HEALTH for Scarborough Team
1 – Full Time, permanent (35 hours per week)
Reporting To: Manager - Community Health Teams (HEALTH for Scarborough)
Purpose:
The HEALTH (H-Health, E-Equity, A- and, L-Literacy, T-Through, H-Health Promotion) for Scarborough team is community-facing program providing access to care in innovative ways. The mobile health unit (MHU) focuses on bringing holistic supports and services based on the social determinants of health to the community to reduce access barriers. The MHU currently delivers harm reduction and mental health supports, diabetes education, and connection to other SCHC programs and services. We are looking to expand our services and locations within Scarborough, which would be the primary focus of this new position. Our community is comprised of low-income families, single-parent-led families, youth, seniors, hidden homeless and street homeless.
SCHC operates with an integrated and interdisciplinary approach. All roles collaborate with and contribute to different teams and programs to support the community's needs. I will act to support the Hepatitis C team in community engagement and bookings.
The MUH coordinator will maintain a full schedule of events for the Mobile Health Unit through partnerships with community groups to bring support and service to the community. They will keep the MHU in good working order with all needed inventory.
Key areas of responsibility:
· Build and update a calendar of events for the van and Hepatitis C team (6 days a week)
· Develop and maintain relationships with internal and external partners
· Maintain inventory for the MHU - ordering and restocking, receiving referrals and creating packages accordingly
· Assist with reporting per partner agreements
· Maintains defensive and safe driving
· Operate ramps, lifts, and assist to secure/move items as needed
· Keeps the assigned vehicle(s) clean inside and outside and in good working order
· Maintain accurate, up-to-date records on trip sheets, vehicle maintenance, fuel purchases, incident reports, accident reports, vehicle condition reports and other records that are requested by the Coordinator
· Perform other duties as assigned that are reasonable within the scope of the job.
Educational and/or Professional Qualifications:
· Valid driver’s license (“G” class) to drive in Ontario Subject to Ministry of Transportation Ontario Driver’s Abstract
· Diploma or degree in community development, social services or health
· 1- 2 years in scheduling, coordinating and implementing events
· 1- 2 years working in community development or with high-needs populations
· Demonstrated knowledge, skills and abilities through 1-2 years of work experience in a similar field
· Comfortable using computers, familiar with Microsoft Suite (Word, Excel, Outlook etc.)
· Sensitivity to and awareness of cultural, racial, socio-economic diversity within the community
· Oral and/or written fluency in other languages relevant to the community the program serves is a definite asset
· Fleet management experience is considered an asset
Significant Working Conditions
· Ability to lift/carry up to 50lbs and move up to 100lbs using equipment provided (i.e. dollies/rolling bins) on a regular basis
· Flexibility of hours –evenings and weekend work may be required
· Flexible working sites – working in the community
· Willing to drive an SCHC vehicle for the purposes of delivering products, ensuring safe operation and maintenance of vehicle according to manufacturer's recommendations
· Independent and team-based work
· Valid driver’s license required
· Vehicle required
Remuneration: $29.66 – $34.63
Pay Band: F
