The Mental Health Counsellor (MHC) is an enthusiastic, motivated, team player and will work closely with our Primary Care Team as part of a dynamic interdisciplinary health care team. With excellent communication and interpersonal skills, the MHC delivers individual and group programming to patients within the FHT and the wider community. They actively contribute to the development, implementation, monitoring, and evaluation of FHT programs and services. Team members focus on providing patient-centered care primarily through brief, short-term counseling. Additional roles include developing and facilitating psychoeducational groups and supporting clients with service navigation. This is a temporary contract position until March 31, 2026, with the potential for the role to become permanent in the future.
About Us
The Ancient Rivers Family Health Team (FHT) is a group of interprofessional healthcare providers associated with the Ancient Rivers Family Health Organization (FHO), a group of 17 family doctors in Renfrew and Calabogie with a combined roster of 13 000 patients on 1 shared EMR (Telus Practice Solutions).
Main Responsibilities
- Provide short term individual counseling services to patients.
- Collaborate with Receptionists to ensure optimal scheduling for patients based on priority, and to provide input into the scheduling process.
- Collaborate with Physicians/Nurse Practitioners and other interdisciplinary health care professionals (IHPs) to manage the needs of the patients, and to facilitate patient-centered care.
- Identify, analyze and provide appropriate treatment and/or referral for patients’ mental and psychosocial health care problems.
- In collaboration with physician, expedite access to a psychiatrist for consultation when necessary.
- Collaborative development of treatment plans for patients.
- Case management, including links to community services.
- Share results of assessment and intervention with referring physicians and other Team clinicians as requested within a specified timeframe.
- Utilize the electronic medical records system (EMR) to record and review information pertaining to patients that can be used for patient assessments and consultations.
- Lead, participate and provide input into a variety of team meetings, projects and committees, as requested.
- Assess and make recommendations regarding programs and services to ensure it aligns with best-practices and the ARFHT’s strategic direction.
- Connect and build relationships with community partners, as needed.
- Update and maintain educational resources to ensure up to date and best-practice information is provided to patients.
Position Qualifications and Skills
- Successful completion of a Master’s Degree in Social Work (MSW) or recognized equivalent and membership in the affiliated professional college required.
- Formal training and experience in evidence-based treatments such as CBT, DBT, ACT, EFFT etc.
- Three (3) years experience working with patients and families in a multi-disciplinary team within a primary care setting preferred.
- Experience in program development and evaluation an asset.
- Experience providing psychosocial assessments.
- Experience providing short term one on one counseling.
- Demonstrated ability to communicate effectively with patients, families, and members of an interdisciplinary medical team.
- Experience with group-based interventions.
- Experience in building community partnerships for the enhancement of patient services and continuity of care.
- Excellent written and verbal communication skills.
- Intermediate knowledge of Microsoft Office Suite and EMR (e.g. Telus Practice Solutions Suite) preferred.
- Demonstrated ability to work well independently and in a team environment.
- Current Ontario Driver’s License and access to a vehicle for travel within the region.
