Position Overview
This hybrid role combines responsibilities as an Intake Coordinator and Family Support Worker. You will be the first point of contact for new families, managing the intake process and ensuring each client is onboarded smoothly and receives the right care. You will also provide hands-on support to children, youth, and adults in our programs.
This is a client-facing, detail-oriented role that requires excellent communication, strong organizational skills, and genuine care for families with complex needs.
Key Responsibilities
Intake Coordination
- Conduct initial assessments with new clients and families
- Gather medical, behavioral, and support documentation
- Coordinate onboarding, care planning, and service eligibility (FSCD/PDD)
- Schedule intake meetings and act as main point of contact during transition
- Maintain accurate and confidential client files
- Provide families with resources and help navigate available services
Family Support Work
- Support clients with personal care, life skills, and community engagement
- Implement care plans and collaborate with families and caregivers
- Document daily care, progress notes, and incidents accurately
- Use behavioral strategies and de-escalation techniques as needed
- Participate in community outings and appointments with clients
- Respond to on-call situations and emergencies during assigned shifts
What We’re Looking For
Required:
- Bachelor’s Degree in Social Work (BSW)
- RSW designation (mandatory)
- Minimum 2 years’ experience in intake, social services, or direct care
- Experience working with children or individuals with developmental disabilities
- Excellent interpersonal skills and emotional intelligence
- Strong documentation and administrative abilities
- Ability to work under pressure in a fast-paced, client-driven setting
- Valid Driver’s License and access to reliable transportation
- Clear Criminal Record Check (with Vulnerable Sector) and Child Intervention Check
Preferred:
- First Aid/CPR certification
- Knowledge of FSCD and PDD systems
- Crisis intervention training or Non-Violent Crisis Intervention certification

Job Types: Full-time, Part-time
Pay: $21.32-$24.34 per hour
Expected hours: 15 – 40 per week
