Intake Coordinator:
MAJOR DUTIES:
The following are the duties for this position:
· Support the Manager with day to day operations
· Arrange and coordinate all aspects of new client intake
· Verify insurance eligibility and benefits and ensure authorization to ensure services provided will covered
· Assist in coordination of referrals and staff’s daily assignments to provide high quality client care
· Act as a liaison between referral sources and our Organization
· Track received, accepted and denied referrals for reporting purposes
· Schedule and conduct end of care interviews and document feedback
· Generate end of month intake statistics and reports
· Receive incoming & send outgoing faxes.
· Receive and register visitors, preventing unauthorized entry via the front door.
· Remain at the reception desk to answer telephones, take messages and/or direct callers to voicemail, as necessary.
· From time to time, to conduct a comparison of stationary suppliers to ensure that we are obtaining the best prices.
· Proactively send out Home Care meeting topic requests too all required staff before the meetings, and ensure all staff receive meeting minutes after the meetings.
