Overview
The Health Care Aide plays a vital role in providing compassionate and supportive care to individuals in various settings, including assisted living facilities, nursing homes, and private homes. This position requires a dedicated individual who is committed to enhancing the quality of life for patients by assisting them with daily activities and ensuring their overall well-being. The Health Care Aide will work closely with healthcare professionals and family members to deliver personalized care tailored to each individual's needs.
Duties
- Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting.
- Monitor and document vital signs and changes in patient conditions.
- Provide companionship and emotional support to patients.
- Administer medications as prescribed and ensure proper medication management.
- Assist with mobility using equipment such as Hoyer lifts when necessary.
- Maintain cleanliness in patient living areas, including laundry and meal preparation.
- Support patients with memory care, including Alzheimer's and dementia care techniques.
- Follow HIPAA guidelines to maintain patient confidentiality and privacy.
- Collaborate with healthcare teams to develop and implement care plans.
- Document patient care activities accurately using Electronic Medical Records (EMR) systems.
Skills
- Strong patient observation skills to identify changes in health status.
- Experience in assisted living, nursing homes, or long-term care settings.
- Knowledge of medical terminology and documentation practices.
- CPR certification and familiarity with catheter care procedures.
- Experience working with individuals with developmental disabilities or special needs.
- Ability to provide hospice care when required, ensuring comfort for terminally ill patients.
- Proficiency in home health care practices including cooking, cleaning, and laundry tasks.
- Understanding of EHR systems for accurate medical record keeping.
- Compassionate caregiving skills that promote dignity and respect for all patients.
