The Chatham-Kent Family Health Team (CKFHT) is a collaborative team of professionals including Physicians, NPs, Nurse Educators, Dietitians, Pharmacist, Occupational Therapist, Social Workers, Chiropodist, a Health Promoter, Certified Respiratory Educator and Diabetes Education Team. The CKFHT is located on five sites across the municipality of Chatham-Kent – Chatham, Dresden, Ridgetown, Wallaceburg and Blenheim. The CKFHT currently has ~35,000 rostered patients.

MINIMUM JOB REQUIREMENTS
Registered Practical Nurse License from a recognized College or University
Must be in good standing with the College of Nurses
Minimum 1 year experience
Accuro experience an asset
Needs to be able to travel to all sites.
Must be Double Vaccinated
TB Test Required
Vulnerable Sector Criminal Check Required at incumbent's cost
KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED:
Ability to maintain quality, safety, and/or infection control standards.
Knowledge of patient care charts and patient histories.
Ability to educate patients and/or families as to the nature of disease and to provide instruction on proper care and treatment
Knowledge of related accreditation and certification requirements
Ability to perform standard clinical procedures according to established protocols
Knowledge of patient evaluation and triage procedures
Ability to perform routine invasive procedures, such as subcutaneous and intramuscular injections in accordance with established clinical protocol.
Knowledge of inventory management practices.
