Overview:
This member of the Assertive Community Treatment Team (ACTT) multidisciplinary team is responsible for assessment of client needs and concerns related to concurrent disorders and setting goals for recovery to promote optional personal functioning and resilience, as well as community engagement by the client, significant others, and the clients family. The Concurrent Disorder Specialist collaborates with the team and clients as well as practices using integrated treatment models in which mental health and substance abuse treatment are provided simultaneously to incorporate within the client’s recovery plan. The Concurrent Disorder Specialist participates in the application of concurrent disorder intervention techniques to assist with psychiatric assessments, assessing clients’ physical health needs, making appropriate referrals to
community physicians, providing management and administration of medication in conjunction with the psychiatrist, medical practitioner and ACT Team Nurses and provides a range of treatment, rehabilitation, and support services. This position reports to the ACT Team Leader
Qualifications:
A University degree or College diploma in a relevant health / social science field
Membership in good standing in an Association or College eligible under the Psychotherapy Act of Ontario (where applicable)
Training and certificate in a potential institution in concurrent disorders
3 to 5 years of experience in mental health / addiction services with in-depth knowledge of complex clinical issues
Demonstrate skills in conducting interview, screening and assessment of needs and mental status
Case management and advocacy skills
Working knowledge of community resources and agencies
Understanding and sensitivity to social and cultural diversity
Ability to work collaboratively in a self-directed work team
Computer literacy and recording skills
Cultural Competence for diverse populations
Able to work flexible hours
Valid Driver’s License
Criminal Records Check for vulnerable populations
Certification in CPR and First Aid or willingness to obtain same

Duties:
  • Adherence to all policies of the CMHA Kenora Branch including assurance of confidentiality of all client, personnel and organizational records and information
  • Maintenance of professional qualifications and registration
  • Maintenance of clients files and data files including updating of client information consistent with policies and procedures of CMHA Kenora Branch
  • Provide an integrated treatment model in which mental health and substance abuse treatment are provided simultaneously
  • Thorough screening and assessment of client needs, mental status and development of appropriate treatment plans
  • Psycho-education regarding mental health / illness and short and long-term treatment plans
  • Facilitate and monitor the implementation of client treatment plans
  • Provide case management for an assigned group of clients and their families
  • Perform shift management as per established policies and procedures
  • Participate in daily staff meetings and recovery plans meetings
  • Participate in training and educational opportunities
  • Participate in administrative and office functions as needed
  • Provide a safe environment by ensuring adherences to workplace health and safety regulations and policies, infection control guidelines, WHMIS and Safe Work Procedures
  • Ensure client safety and client safety activities are emphasized throughout orientation of new staff members
  • Support an environment which avoids, prevents and corrects all activities or actions which may result in an adverse outcome in the delivery of mental health services
  • Introduce concurrent disorders to clients, orient the client to concurrent disorders and its rational; assess client needs and concerns; set initial goals for recovery with client
  • Perform assessment tools related to concurrent disorders
  • Facilitate understanding and application of concurrent disorder intervention techniques
  • Re-assess goals/treatment plan, refine/continue intervention techniques
  • Educate clients to identify early warning signs and prepare to maintain changes
  • Facilitate development of self-management plan
  • Evaluate feedback from clients and use of clinical outcome measures
  • Responsible for self-care
  • Other duties as directed
