About the role: Under the supervision of the Service Director and the immediate supervision of the Residential Coordinator or their designate the Community Reintegration Worker will perform various tasks related to client care, activity monitoring and general program support.
As the successful candidate, you have:
- Education: University Degree or College Diploma in a social services program (psychology, criminology, social work, social service worker, community justice worker, developmental service worker)
- Required Certifications: Current First Aid/CPR/AED (Level C)
- Current or recent Non-Violent Crisis Intervention & Physical
Intervention Skills (NVCI, CPI, UMAB, PBAM, TCI, Safe Management, etc.)
Other certification and trainings may be considered an additional asset
- Experience working with individuals with dual diagnosis navigating the justice system is considered an asset
- Familiarity with specialized approaches for supporting clients with complex needs, such as developmental disabilities, mental health, addictions, homelessness and trauma.
- Ability to coordinate all on-site program related functions through a gender- and trauma-responsive framework
- Excellent people skills to facilitate relationships with clients, colleagues, supervisors, program partners and community members
- Preferred experience: Previous experience in a direct support role, supporting individuals with complex needs. A minimum of Two (2) to Three (3) years of preferred.
- Language: Bilingual preferred (English/French). Other languages will be considered an asset
You’ll also be prepared to:
Direct Support
- Support individuals with completing activities of daily living (ADLs/IADLs), including direct support, education, and development of life skills, and coaching
- Administer medications with precision and adherence to established protocols and guidelines
- De-escalate crisis situations with a calm and effective approach, maintain respect and dignity for the individual and ensure the safety and wellbeing of all involved
- Implement individual support protocols identified in Individual Support Plans (ISPs) and Behavioural Support Plans (BSPs) with strict adherence and precision
- Act as a positive role model for clients, providing impactful life skills training and behaviour management
- Prepare accommodation spaces/units for new arrivals and participate in the orientation to the program with the individual as directed/required
- Engage individuals with prosocial activities and resources in the community that align with their interests and goals
- Provide community accompaniment and supervision to individuals with imposed release conditions and/or high support needs
- Utilize community resources to support individuals with their goals and interests
- Familiarize individuals with community resources and amenities

Community Integration and Support

Foster the development of essential community living and life skills, with emphasis on those that contribute to meeting release conditions and addressing identified risk factors

Familiarize individuals with community resources and amenities
Develop and maintain up to date knowledge of community resources and services appropriate to individuals served
Liaise with external service providers and connect individuals with
Attend, participate and facilitate educational events and trainings with external resources
Administrative Duties
- Complete shift/daily log notes, daily checklists promptly, ensuring accuracy and up-to-date records
- Adhere to quality assurance measures (QAM), ensuring accuracy and compliance with established standards and related administrated tasks
- Respond to inquiries via phone or e-mail, provide appropriate information and communicate effectively with all team members and management
- Complete behavioural data collection and incident tracking
- Complete Serious Occurrence Reports (SORs)
- Complete inventory and packing of individual belongings when required to do so
- Attend and participate in team meetings and trainings as directed
Building Safety, Maintenance and Cleanliness
- Complete routine household chores and minor repairs to maintain a well-kept and functional living environment and workplace
- Monitor the building and premises to promptly identify any safety concerns or issues
- Participate in and execute emergency procedures effectively
- Ensure cleanliness of the building and property to maintain a healthy and hygienic environment
- Ensure compliance with all Quality Assurance Measures (QAM) related to building cleanliness, safety and maintenance
